Intra-medullary lesions first involve the cervical fibres in the Corticospinal and spinothalamic tract and then involve other fibres in a descending fashion. That's why there is descending sensory motor symptoms in Intra-medullary lesions

I also came here to rectify this doubt. As we can see intramedullary expands all around so resulting in bilateral lesions and extramedullary are localized resulting in unilateral lesions

Sir, my last MRI report in 2018 showed intramedullary spinal cord lesion C2-C4, a small lesion on the pons and cerebellar atrophy and no one has discussed this with me and what it means? This came on after my cervical corpectomy on levels C5-C7 in 2016. Do I need to see another spine surgeon, neurosurgeon or neurologist?
