Never - the guy is like a machine. He demands perfection from the musicians around him, but he delivers a rock solid performance himself every time. Say what you will about the guy, when he hits the stage there is no one more professional.
I don't know about that, my friend. I was listening to a recent Priest gig and Rob seems to be letting the audience take it on more and more high parts. I don't think they even do Green Manalishi any more - but I could be very wrong and I could have just heard an off performance. Gotta love Priest -

Gene’s voice has held up remarkably well, particularly considering he always (live anyway) sang with a sort of Cookie Monster growl to his voice, and he pretty much gargled with kerosene every night on stage, his vocal is still spot on and every bit as nuanced as it was in the 70’s
That was really killer! Gene just has a certain something that makes him just rock out there! Also, that drummer is a fucking monster!! What a killer basher!!
