  • Identifies self by name. Identifies resident by name.
    Washes hands.
    Explains procedure to resident, speaking clearly, slowly, and directly. Maintains face-to-face contact whenever possible.
    Picks up diet card and asks resident to state name. Verifies that resident has received the right tray.
    Raises the head of the bed. Makes sure resident is in an upright sitting position.
    Adjusts bed height to seat self at resident’s eye level, locks bed wheels, when assisting resident in their room.
    Places meal tray where it can be easily seen by resident, such as on overbed table.
    Helps resident clean hands if needed.
    Helps resident to put on clothing protector. If refused, must be on care plan. All residents must be offered a protector.
    Sits facing resident, at resident’s eye level, on resident’s stronger side. Never feed resident standing up.
    Tells resident what foods are on plate and asks what resident would like to eat first.
    Offers the food in bite-sized pieces, telling resident content of each bite offered. Alternates types of food offered, allowing for resident’s preferences. Makes sure resident’s mouth is empty before next bite of food or sip of beverage.
    Offers drink of beverage throughout the meal.
    Talks with resident during meal. Does not conduct personal conversation with other staff while assisting the resident to eat.
    Wipes food from resident’s mouth and hands as necessary during the meal. Cleans resident’s face and hands with warm soapy water.
    Removes and disposes of clothing protector if used. Makes sure all assistive devices are free of food particles before leaving dining room.
    Removes food tray, checking for personal items. Places tray in proper area.
    Returns bed to lowest position.
    Places call light within resident’s reach.
    Washes hands.
    Documents meal and fluid intake.
