My issue with this is that Imodium primarily acts by slowing down gastrointestinal motility, which allows longer dwell time for bacteria in the gut to proliferate and excrete the same toxins causing the diarrhea. This is why Imodium is contraindicated in infections like CDIFF and in conditions like toxic megacolon. It's also why, in my opinion, it shouldn't be used when there is an acute unknown bacterial infection causing the diarrhea. While Pepto-Bismol's mechanism of action is poorly understood, it does have *some* ability to affect bacterial binding to the intestinal mucosa and is more safe in general when used as directed. I don't think pepto-bismol should be used with oral antibiotics, as you stated, but as a stand-alone drug it is clearly the better choice.

So should you or can you take the Imodium or Pepto-Bismol before you eat something or take it after?
