The problem is that half of the doctors railing against cheek implants will be fans of fillers (Dr. Prasad), while the other half will be fans of fat (Dr. Karam). Then, of course, there are doctors who love cheek implants (Dr. Niamtu) as much as you hate them. Every method has as many supporters as it has detractors, even among professionals who you'd think actually KNOW what works best and what doesn't work. And they don't even have a discussion about it, where one responds to the arguments of the other. We just have a collection of stand-alone statements of surgeons telling us why method A/B/C is good and everything else sucks. This makes this field impossible to navigate.
But fillers aren't any better! You really look puffy over time with them, and they migrate. I'm getting jowls and I really have a flat mid face, so I'd really like cheek implants.
Yes exactly. if you had weigh the pros and cons, implants outweigh fillers etc everytime...fillers will destroy your face over time and have to worry about about getting them every couple of months or so...fat filler will start to sag over time
During my personal journey with improving my looks i considered custom cheek implants to get defined cheekbones, but then, after looking at 10/10 people a lot and studying it, i realised that real cheekbones are not necessarily big, but they are grown forward together with the lower face, in a way that makes the cheebones protrude on the upper side, but be flat on the lower. So a cheekbone implant could never achieve that because it is protruding forward independent of the lower face, result in it looking unnatural because it protrudes on the lower side as well. if that makes sense. but fillers are even more trash, they do literally nothing except make you look like your face is bloated permanently.
As someone who’d love implants but decided against it, it’s not permanent as no implant is and as we age we lose bone density which causes issues for any implant as we age.
I have an extremely thin face and frustrating hollows, I get cheek filler but it ends up migrating and making me look like I have droopy puffy lower cheeks with completely flat middle cheek and upper cheek/cheekbone so I just look like I have some ugly pouches drooping down. It only looks good for the first week when I'm swollen. I am like tapped out money wise spending $1000 on filer every month because my body just metabolizes it so dang fast and or it migrates and looks so weird and droopy 😭 I don't really have any fat to do a fat transfer so I'm running out of options
im in the same situation! my face is super thin and i have hallow cheeks im looking like a skeleton at 28 my body goes through filler very quickly & the fat from the fat graft didn‘t last.. ughh im really thinking implants are my only choice! srsly i can‘t look any worse than i now do 😢
But what about the downsides/risks of autologous fat and dermal filler? Filler usually has to be repeated every 12-18 months or so (sometimes sooner depending on the individual) to maintain the effect which is not a cost-effective option, long-term. There is a risk of bruising, bleeding, infection and, more seriously, occlusion and necrosis every time you are injected. It's almost impossible to get the exact same aesthetic outcome consistently every time you 'top-up'. Filler can migrate - even when placed correctly and, as demonstrated by MRI scans, filler can stick around a lot longer than we thought in random parts of the face so it's less noticeable, leading to potential complications like chronic edema and an overall puffy look that so many people have these days. Autologous fat transfer seems like a better option in some ways but there is limited precision in terms of knowing how much of the injected fat will stay and how much will be reabsorbed into the body, potentially resulting in asymmetry/over or under-filling, and requiring additional procedures. Fat injections can also form visible and palpable lumps, particularly under the thin skin of the tear trough area, that have to be surgically excised. Is it not possible to customise each implant so that the overall effect is ~symmetrical~ on both sides and positioned in a way that is more natural? Do all implants cause bone erosion? Why does aging make implants look worse?
Fillers migrate, and they look heavy. Moreover, not all fats survive so you get some irregularities too. So what's left? Facelift? You may not even have enough fats to lift, which is in most cases.
Fat transfer or grafting does not last at all for most patients period. It may last if y9u do it monthly for a long time but definitely disappear in most. So there are pros and cons. I think face fillers are best from my own experience
What about patietns with underdeveloped jaws, jaw surgery can only reach so far up and for example narrow maxilla causes flat and unprojected cheekbones. Would those patientsa be ideal candidates for implants? Filler is nice but it doesnt resist gravity like an implant
Fillers are not any better... they cause buildup over time and are not always readily absorbed. Dr Rohrich; what are your thoughts on PEEK implants for cheek?
At 1:11, I didn't get the point of "they did not move, and looked unnatural". Isn't an implant supposed to imitate bone? BONE should NOT move. So, i don't get the problem here.
At least Amber can take them out should she so desire. I have this same type of cheeks naturally. They are so prominent and when I lose a few pounds I look really weird. Thinking to get some volume to my temples to soften the harsh look.
Amber’s are not implants because they go up and down. I’ve watched her. She gets them done and they look huge but after a while they flattened out and then she gets it done again.
After having double jaw surgery to align my asymmetric bite, I have one cheekbone that’s much weaker than the other. Would this be a scenario in which you think a custom cheekbone implant would be a good idea if it also covered the lower eye and temple area?
When you age and have loose skin you need a facelift. Don’t have implants or fat added. I’m not a surgeon but common sense and some education makes this obvious to me. No fillers or your own fat no .
They can work but there’s so many better options today including using longer lasting fillers like Radiesse along the mandibular border that don’t have any significant morbidity long-term like jawline implants
I agree. cheek plants are the #1 thing to make a person whose getting work done suddenly look alien like. it also ages the crap out of people. I mean amber looked gorgeous before she got this done. Now she looks creepy
No, Amber's cheeks looks great, theyre small subtle and most people would never know. i have a friend with small ones in her 50s and she looks great! There are some beautiful cheek implants done on flat faces on Real Self, huge difference and great improvement. Much better than filler which gravitates over time and people look "pillowy".
