Hello DR. My baby was born at 37 weeks (on June 9th), and she is currently 35 days old. On day 4, we noticed jaundice for the first time at 17.2. The gynecologist suggested phototherapy in their hospital, but I believe it was ineffective because of a problem with the device. On day 6, the jaundice value increased to 22, so we took the baby to a children's specialist hospital, where the doctor suggested double phototherapy using two devices. On day 7, the values were at 14.4, and on day 8, its was 11.5 so the doctor suggested that the baby be discharged as other diagnoses, such as "Coombs test." was normal. After three days (Day 10), we saw that the eye was yellowish. The value was 19.6, and we were advised to undergo phototherapy for the next 48 hours. On Day 12, the jaundice level was 8.3. On Day 16, we noticed the next re-bound, with values standing at 14.8 and 15.2 after two days (phototherapy was not administered). on day 18 we again started the phototherapy for 48 hours, as of day 20, the jaundice was 9.4. and we had also performed thyroid, TMC, and ultrasound tests, all of which were normal. On the 30th day, the jaundice re-bonded at 15.4 , and the doctor advised stopping the Brest feeding to rule out if that is the source of jaundice. We tested the jaundice on the 34th day, and for the first time without phototherapy, the jaundice is under control at 11.7. Therefore, I have stopped the Brest feeding for a week, and I do not see any high symptoms of jaundice again for next one week (Till today). Would like to know when I can begin Brest feeding and how often we should jaundice test to ensure there is no rebound. I am eager to continue the Brest feeding, so please advise me on the best course of action.

Hi Doctor. My baby is 27 days old, he was on phototherapy from day 3 to day 5 and then from day 7-9 and he was off it. Now on day 25 his reading is 250 total (direct 21.4) and he got admitted again but no phototherapy this time. And on day 26 his total is 238 (direct 18.6). His blood results, culture, and thyroid test is all good and no abnormalities found. He is breast fed completely and his diaper count is good and stool is mustard yellow colour. Doesn’t have temperature, but his breathing is on high side. It measures 180 when he is feeding/crying and at rest it is 155. He has some congestion and doctors told to give saline drops before each feed. He was 3.1kg when born and now he is 4.1kg. We are worried about kernicterus. We are now being discharged by nhs and no follow up given. What should we do doctor?. Should I get the levels tested every 2-3 weeks? Thanks. G
Yes, you don't need to worry as it didn't rise to very high levels-kernicterus won't happen below 350-400 at least. It could stay around this level for another few weeks but since all work up is normal I would just repeat after 10-14 days first and then repeat till almost normal (every 2-3 weeks)

My baby was born at 36 weeks to GDM mom. Mom blood group is B+ and baby's AB negative. Bilirubin was 16.4 on 3rd Day of life. Direct was 1 or less than 1. Predominantly it was unconjugated one. G6PD, Thyroid was all normal and ruled out. Its been 40 days now. Phototherapies would lower it....But after sometime, it again rises gradually, like 1 mg/dl per day. Dr had then stopped breastfeeding for 24 hrs. The level has reduced to now 8. Baby's birth weight was 3.195 g. Dr has called us after 1 week. After 24 hrs breastfeeding cessation, Baby isn't taking it well, and seems to have gas. Cries a lot. Urine is colourless. Stool had stopped for a week .Now it has resumed. It was dark but yellow now. What is the treatment for this breastmilk jaundice, if it is indeed a breastmilk jaundice?
Since bilirubin is only 8 now, and hopefully we have a recent direct bilirubin which is normal (if not, do it as well next week)- you can go back to full breastfeeding and just repeat next week and then every 2-3 weeks till normal
Hello sir. My baby is 90 days old now. He still has breastmilk jaundice. He has been in phototherapy twice. I stopped breastfeeding for a week and his values decreased from 14 to 9. Its concluded that breastmilk jaundice but still i got all his tests done CBP, thyroid, LFT, G6PD, all are normal. Now what to do? When will his levels reduce? He's already 3 months old. Pls reply.
What are the last results for direct bilirubin? Is Hb normal? There could be associated Gilbert syndrome which doesn’t affect either. Since all results ok, continue to monitor 2-3 weekly include total direct bilirubin and ensure you did free T4 and TSH as well

Hi Sir! My baby was born in 37 week gestational age on 15th December, 2023. She still has jaundice. Baby's blood group-AB+ve, Mother's blood group-B+ve, Father's blood group-A+ve, TSB-11.39 (direct-1.16)mg/dl, TSH-6.18, Rectic count-4.6, DCT-negative, HB%-8.6gm%, LDH-233, peripheral blood smear-no abnormal cells detected, Iron-92.8, Ferritin-345.5. Can breastmilk jaundice be the cause of high bilirubin with low hb% count but normal iron and Ferrritin level? She is around 7 week age now.
Yes, it’s likely breastmilk jaundice. All babies have physiological anemia at this age and will soon increase on its own so continue to repeat after 2 weeks (retic count increasing means baby is already producing red cells so likely next Hb will be higher). Don’t worry, repeat total, direct bili, TSH and free T4, full blood count in 2 weeks

The follow up test was done on 22nd - 13.2 and the next test was done on 24th - 18.46 . He was admitted at this point for 3 days for phototherapy after which his bilirubin level came down to 6.x and he was discharged. All this time he was always breast fed as I am having enough supply of breast milk. On 3rd august his test was done again as he was again turning yellow. this time the level was 13.6. doctor advised us to admit again and asked us to give him formula milk as he suspected breast milk jaundice. After one night of phototherapy his level came down to 7.8 . Coombs test and ast alt tests all are negative and under control Can you tell me if the recurring jaundice is really due to my breast milk? Do I need to only give him formula milk henceforth?
It is likely breast milk jaundice but nothing to worry, it’s very unlikely to need treatment from this stage, cut off for phototherapy is 22-24 mg%, it will stay in this 13-15 range for a few weeks and drop. Check after 3-4 days and if stable, check every 10 days till below 10 and then once more after a month. Consider urine analysis and culture if rises again

Hello sir, my baby was born on 13th July 2023 at 37 week gestation. his first bilirubin test was done after discharge on 17th July - 11.8 reading. Coombs test and ast alt tests all are negative and under control
Can you tell me if the recurring jaundice is really due to my breast milk? Do I need to only give him formula milk henceforth?

Also his direct bilirubin test is normal in all the reports and thyroid test done at birth was also normal

We did nothing but gave her sunlight on daily basis and a bit of phototherapy tried myself with white tubelights as doctor said it is not too much and will go away with home treatment also gave dexatop water one ounce daily for three days. Repeated her test after 26th day of her birth and her bilirubin was 13.9. Now drs are asking me to stop breasfeeding her as it could be breasmilk jaundice because of prolonged duration and give her formula milk with colibid drops thrice and protectis drops once daily. Pls tell me should I stop feeding her completely or I can give her 30% breasmilk and 70% topfeed. Since she also takes top feed but the ratio was opposite before so its difficult to instantly stop breastfeeding her. Also tell me what to do this time?
Thryoid was done at the day of her birth which was normal.
Blood group A+of both mother and baby.
Weight gained 1 kg in one month duration. Pls tell me what to do and what's the possibilities. I am very confused
@@EemanSalman did you check direct bilirubin as well, and is stool color normal? No need to disrupt breastfeeding, no formula needed, but repeat total and direct bilirubin as well as free T4, TSH in a week
Thankyou very much for your reply! It was SBR test. Stool is yellow everytime and passes her stool 5 times a day.
@@EemanSalman ok, do as above
What’s this dextop? Hope not a steroid?
thank you dr.
You are welcome

My question is we have given her proper checkup from when jaundice started, UDILIV, LIV52 AND SARBOLINE DOCTOR GIVEN ME MY WIFE ALSO WE HAVE ADMITTED ALSO ON 6 TO7 MONTHS OF PREGNANCY. WHAT JAUNDICE NOT GOING, 1ST QUESTION
2ND ISSUE Age 3day weight 2.3kg
BABY DELIVERED NORMALLY.
BUT HE ALSO GOT JAUNDICE + INFECTION CALLED CMV & Rubella.
He is under light therapy (i dont have enough money but i am doing my best)
Now Doctor giving him

NOW MY QUESTIONS IS SHOULD WE TAKE OUR BABY AND DAILY TAKE HIM INTO SUN LIGHT Will everything fine?
2nd :- Mother also has Jaundice so she can't brestfeed right?
If she can feed that boy will become normal soon.
I tried searching everywhere but i didn't found that both mother and baby has Jaundice at the same time.
I'll pay you if you want (if you can help me without payment that may help me)
what should i do in this situation?
i am very worried and not able to sleep properly in this situation
B positive blood group both
The mother’s problem is most likely cholestasis of pregnancy, did she have itchy skin too? Hopefully will improve now. Baby’s jaundice is not related to that, likely to improve soon, don’t worry. Doctor might have done tests for CMV and Rubella as screening for small size. Doesn’t mean baby has infection. As for treatment, follow your doctor advice Best wishes
@@SridharKs yes mother has itchy skin that time, But Jaundice still there can she brestfeed baby?
baby rubella And CMV report
CMV IgG (Serum CMIA) - Reactive 776.90 Au/ml
Rubella IgG CMIA - Positive 54.50 IU/mL
