It's a very technique sensitive block. Don't get disheartened if it doesn't work. Palpate the anterior border of mandible and then direct it from the opposing premolars and pierce it right at the center untill u feel the bone, once reached, retract the syringe, aspirate it to check for blood. Inject if negative. Then direct the needle towards the same side molars while retracting, again aspirate and then inject for lingual nerve block

Hi doctor one common problem I am facing is that sometimes even after successful inferior alveolar nerve block the pulp is not anaesthesised.... patient feels pain during RCT... could you suggest what to do? Thanks...
@@Saidentalvlogs no doctor during access opening only I face this problem... yesterday only I encountered this problem...I stopped the procedure after a couple of futile efforts. I put formocresol open dressing and put the patient on antibiotics and recalled her after 3 days...but the entire incident was quite humiliating and disappointing...
@ Paramartha Ghosh The tooth can be infected and result in a hot pulp. Which may be painful as local anaesthetic doesn't act well locally in the area of infection. You can add antibiotics and try extirpation the next day. Or secondly the block may have failed. Check to see if the block is working accurately.

if aspiration is positive means we have pieced a blood vessel, so we have to discard the syringe and try again with fresh one

Dada,according the malamed ,the height of insertion is 6-8 mm above occlusal plane.it is lil bit confusing.
Plz follow the landmark as said in the video, practically it works well.
The occlusal line is best for Closed mouth technique to anesthesize IAN.
