I'm not sure about the 'two women at once' idea. He had the chair made because he got very fat and therefore found it difficult to 'perform' in the conventional position. He was able to stand at this contraption, with the woman lying on the top deck with her feet in the stirrups. There's no way he could've got down to floor level if there was another woman lying down there! More likely, I would've thought, he would be the one to lie down there after his exertions!
Are you questioning the performance? Dude was a frequent customer. He must have a lot of stamina... 👀
