  • Pyoderma gangrenosum (PG) is a rare and often misunderstood inflammatory skin condition. It is a type of neutrophilic dermatosis, which means it is caused by an abnormal immune response involving white blood cells called neutrophils.
PG typically presents as a rapidly progressing, painful, and ulcerative skin lesion. It can occur anywhere on the body but is most commonly found on the legs. The skin lesion may start as a small bump or pimple-like lesion and rapidly progress to a painful, deep ulcer with raised, bluish-purple edges. The ulcer may also have a characteristic "undermining" or tunneling appearance, which can make it difficult to heal.
The exact cause of PG is unknown, but it is believed to be related to an abnormal immune response. It may be associated with other underlying medical conditions such as inflammatory bowel disease, rheumatoid arthritis, and blood disorders. PG can also occur spontaneously without any underlying medical conditions.
The diagnosis of PG is often challenging and requires a thorough medical evaluation and skin biopsy. Treatment typically involves a combination of topical and systemic medications to control inflammation and promote healing. Systemic medications may include corticosteroids, immunosuppressants, and biologic agents. In some cases, surgery or other interventions may be necessary.
It's important for individuals with PG to work closely with a healthcare provider who has experience treating the condition. With proper diagnosis and treatment, many individuals with PG are able to achieve significant improvement in their skin lesions and quality of life.
