the exercise you show here Dr. Hesch is if only the lower quadrant on the left and right sides of the sacrum is prominent or is in an torsion? What about the upper quadrants of the sacrum; would there be a different exercise? Or maybe this exercise is treating the whole sacrum?

I can feel when it has moved out of place and it’s terrible. My ligaments stretched and now I have to keep putting it back in place. It’s fine when I get it there but keeping it there atm is difficult. Stops you bending when it’s out. The movement feels horrible. You can feel the bones sliding over each other until they are seated correctly and when unseated it makes all of the spine unstable and move around.
It depends on a thorough whole-body evaluation. I test and treat micromotion defects throughout the body. I occasionally use heel lifts and ischial lifts if the pelvis grew uneven the person will be uncomfortable while sitting.

the sacral torsion is a form of sacroiliac joint dysfunction. You can share my videos on self-treatment for sacral torsion with your physical therapist to see if they can guide you. Depends on your job and sport if very demanding then daily otherwise 2x week should suffice but clarify w your healthcare provider you can share the video. I cannot know if it is appropriate for you or not

No, it works for 4-types of sacral torsion, there is no such thing as "unilateral flexion/extension of the sacrum, does not work for sacrum stuck in backward bending. I treat sacrum that lacks backward bending with inferior glide and implicate the lumbosacral facets more so than the SIJ (but who knows it has not been specifically evaluated), maybe it works for the latter.
