CASES 1-20
The DBR, presented by Clay Cockerell, MD, is a comprehensive online review that is designed to prep participants for the dermatopathology component of their board or recertification exam. This is accomplished by reading approximately 60 e-slides, taking a board similar quiz, and conducting an in-depth video review of each case. As a bonus, there are an additional 40 self-paced cases with an in-depth answer key.

3:57 - Porokeratosis ( cornoid lamellar + band like lymphocytic infiltrate)

10:09 - PLC ( it never has eoisinophils, DDx: EM is acute do not has crust, PR may has extravasated RBCs but no interface dermatitis)
13:30 - FDE ( dark colour on low power= lymphocytes, deep melanophages= recurrent episodes at the same site, dyskeratotic KC,
17:36 - PRP ( psoriasiform hyperplasia of the epidermis with bulbous rete ridges, no diminished granular layer, no neutrophils, no eosinophils, plugged follicular oppining, alternating parakeratosis= checker board) PRP may have foci of acantholyisis but not psoriasis
23:50 - Pellagra ( late stage of it may resemble psoriasis) , coxacky viruses may cause this kind of balooning
29:15 - Psoriasis ( this one has irregular eidermal hyperplasia= rapidly developing lesions= pustular), SD does not have neutrophils
33:48 - AA ( decreased density of follicles, normal is 8-10, here there is 2, peribulbar lymphocytes)
39:18 - Sweet ( prominent papillary edema, dense infiltrate of neutrophils, minimal involvement of epidermis)
42:49 - LCV ( from low power: exravasated RBCs, perivascular infiltrate, heavy nuclear dusts, fibrinoid change in vessle wall), PPD has no neutrophils, septic vasculitis may has neutrophils but no leuckocytoclasia, thrombotic event has no inflamation
47:55 - Bowen= SCC In situ ( on low power: sun damaged skin, intraepidermal proliferation of atypical cells that forms nests with pagetoid morphology, stains + for p63, DDx: melanoma, LCH, MCC, sebaceoys carcinoma, paget disease, MF), LCA= leukocyte common antigen= CD45
53:18 - Warty dyskeratoma ( single lesion with acantholytic dyskeratoma)
59:10 - Hailey Hailey
1:05:43 - DH ( in subtle cases the collagen may look basalophilic)
1:09:57 - accessory tragus
1:12:43 - PXE ( calcified= basophilic coiled elastic fibers, von kossa stain is useful)
1:16:44 - Lichen sclerosis et atrophicans
1:19:33 - Baloon cell nevus (nesting is prominent here), DDx: clear cell syringoma ( no ductal differentiation here ), metastatic renal cell carcinoma ( no abundant vessles or mitosis here), Xanthoma ( no foamy cytoplasm here), Clear cell sarcoma
1:24:36 - Necrobiosis lipoidica ( palisaded granuloma, lymphocytes&plasma cells, layered pattern, degenerated collagen)
1:28:35 - Eruptive xanthoma ( foamy cells + extracellular white materials= lipids, this finding is not seen in regular xanthomas)
