You should always use anti-seize with spark plugs or O2 sensors as well as dielectric grease for the pigtail connectors. Also change them in pairs. Just did them both today on an 04 i6 LS.
Yes. The oxygen sensor monitors the air fuel ratio and makes changes to adjust to run efficient. But a oxygen sensor DTC could be cause many reasons, such as a misfire, exhaust leak, etc. Further diagnosis is required, with a scan tool you can monitor the live data. In this case, guy bought the oxygen sensor and wanted it replaced, so I did. There was a DTC for it and after replacement DTC cleared, and after a good test drive DTC never came back.
I believe 2. Heated O2 sensors fail naturally. But oxygen sensors will set code(s) due to a lean or rich condition or a failed O2. Recommend checking live data with a scan tool or check for oblivious signs during inspection.
Make sure you warm the engine to operating temp, at least 180 degrees. Use a good socket and a ratchet with a pipe for torque. You cant really get at that sensor with a breaker bar on these engines. The A/C line and a coolant hose are in the way. I changed both in Autozones parking lot today. $57 each for AC Delco.
