i think this is my first time rewatching this episode after reading the novel. I havent seen the film, but I love this episode's Chief Bromden and how they recreated the escape. Frank trying to lift a big metal box and getting Chief to do it. It's perfect. Now I wish they had an entire episode with Frank in a mental institution and had just all the references to Cuckoo's Nest. The episode would end with Frank and all the inmates beating up Ratched or something fun like that

The whole Pepe Silvia rant is one of those It's Always Sunny scenes that I can pretty much quote from memory, and I love the way it ends with the "Barney" reveal - Charlie Day's delivery of "You don't se-oh shit, where did he...?" cracks me up every time. Yeah, it's consistent that Charlie can be a good, hard-working employee but he spirals out of control because the gang are so lazy and incompetent that he has to do all their work for them. Mac in this episode didn't do a single thing to help him so he had to run the whole mailroom by himself.

It is really funny because I grown up in China, sometimes I don't get the references of certain things. But I recognized the reference of 'one flew over the cuckoo's nest' immediately and surprised the crews didn't... 'one flew over the cuckoo's nest' is one of the most famous western novel in China and the movie was actually played by Danny DeVito himself...
Not only a nod to One Flew Over the Cuckoo's Nest, but they also used the theme song. Nurse Ratched, the Chief, the water fixture, taking daily pills, etc. All good stuff.
The thing that cracks me up the most with the mail room scene is Barney just suddenly showing up. He's so mysterious, he's so aloof, ready to hand out ciggs to those in need, it's just hilarious to me.

It's not just Europe, but Australia too. I recently broke my collar bone and the whole process from the ride in the ambulance to the hospital visits to the actual surgery itself didn't cost me a cent.
And up here in Canada. As a matter of fact, most Middle Eastern ,European and Asian countries also have free health care... its really just Americans that have been brain warped into thinking its "evil"... so bizzare.
Here in the US, it's sadly unlikely to ever change. Big pharma, hospitals, insurance companies, government taxes, medical suppliers, etc... there's just too much profit in it. There's constant lobbying from those groups and others to actively make regulations worse, not better.

He was a guest in Always Sunny Podcast and they talked about it! He was supposed to be a new character and they wanted to cast this actor again because they thought he was very funny. The actor even said that when he went to the audition for the new character he was really worried that the creators forgot about him being in the show previously and tried to warn the crew like "Hey guys, just FYI I already was in your show several seasons earlier" and Rob, Charlie and Glenn were like "Yeah, dude, of course we remember you, we want you again". They also said that while the Waiter was supposed to be a different character, they really enjoy the theory about him being fired because of Mac and Charlie and have it in the back of their minds (they also speculated about how it would be fun to make the HR guy and the waiter twins and play with this)

I went to The emergency room in November even with my insurance from my job I had a $833 bill from the hospital. Then I got a letter that said I had to pay $796 for the doctor that saw me. Healthcare in the US sucks!

The USA healthcare system has severe problems, mostly relating to subsidies ( by the way the USA somehow manages to spend more on healthcare per capita) in order to bribe your employers to chip in. People don't really understand that you don't pay 70 dollars a month for healthcare through your employer in the US you pay like 400. They just only "take" 70 out of your pay. The company matches some of the rest and a lot of the rest of that is paid in subsidies to the insurance companies. It's a mess of a system full of grift. But it's not the fact that it's "free market" (It ain't, not by a long shot, look up "certificates of need").

There's no upfront knowledge of what the price will be, so you never know the amount until after. But, since most hospital visits are time sensitive, they know you don't have time to exactly 'shop around' either, so just do it and bill you whatever they feel like, after. Huge profit to be made for them that way.
They don’t tell you to “get out” if you don’t have insurance. In Dee’s case, heart attack, hospitals are required by law to give treatment. EMTALA is the law. They will however be billing the patient directly at full price.

In Brazil we have both public and private systems. If you want better treatment have a private insurance. But if you are poor you won't die and won't be charged for an ambulance ride or analgesics when you get kidney stones. Even having private insurance sometimes I went to the public hospital to get analgesics due to kidney stone pain cause in the private system I'll still be charged something even though I had an insurance. I think neither system is perfect, so having both is better because you have choices. And sometimes the public system pays for a private hospital which had to treat you during an emergency.

But if you are poor you won't die and won't be charged for an ambulance ride or analgesics when you get kidney stones. Even having private insurance sometimes I went to the public hospital to get analgesics due to kidney stone pain cause in the private system I'll still be charged something even though I had an insurance.
I think neither system is perfect, so having both is better because you have choices. And sometimes the public system pays for a private hospital which had to treat you during an emergency.
