Misinformation, the size 2 fish is in the game's files. It's just a very, very low chance. Hakita and PITR are hardcoded to the leaderboard but that's because the chance is so low they didn't want to bother catching it legit.

The size 2 fish is only able to be caught in P-3 outside the map in the river. Since you guys know already that P-3 is just a boss rush with a new boss at the end similar to the Pantheon Of Hallownest AND you know that the boss isn’t a prime soul, I'll make sense of it by saying that the Size 2 Fish is instead the prime soul of the level. To trigger the prime soul bossfight, you have to use the new boss's heart as bait for the fishing rod found under the bridge near the start at the river of blood.
How to get size 2 fish: Step 1: build a particle accelerator Step 2: find 2 fish (size 1) Step 3: put fisj in particle accelerator Step 4: ram the 2 fish against each other at the speed of light Step 5: you now have size 2 fimsh
