For example, let us say the total number of axle repetition of tandem axle for top down cracking is 2743873 Axle load data shows that 14.5 % of total tandem axle in the axle load class of 380 - 400 kN is 14.5 percent. So expected repetition for this load class will be 14.5 percent of 2743873 = 397,861. Hope it is clear now.

The average number of axles per CV will depend on the traffic composition. I mean the propostions of trucks with 2 axles, 3 axle, 4 axles and so on. Higher is the number (or proportion) of the trucks with more number of axles (3 axles and more) greater will be this value. It is not fixed rather it is to be calculated from axle survey data.
It is the total number of axles divided by the total number of CV. For example there are 100 commercial vehiclea and out of it 60 are two axle trucks, 30 are 3-axle trucks and 10 are 5-axle trucks. Now total number of axles are 60x2 + 30 x 3 + 10x5 = 260 axles. The average number of axles is 260/100 = 2.60 Hope it is clear now.

Yes, you are correct. I checked it agian, there should have been (-1) in the numerator. But the calculation is made with (-1) only and therefore, the value of C is correct. Thanks for pointing it out.

only the last one 1.222 is the tridem axle here. Consider it tridem only even if one axle is raised, it is temporary.

Not by IRC 58. You need an axle load spectrum.
If it is not available, assume some based on past data
If we have to design rigid pavement over damaged flexible pavement surface do we have to take DLC below PQC
