"She learned from Terney and still turned out to be a good cop." Terney was a good cop. Go back over his appearances, he was doing his job. That is why he was effective and why he was recruited. HR was a criminal organization, but they also were rather effective for a long time. One reason was they stayed unknown for the most part but having effective police officers who did not draw attention to themselves which might trigger an investigation by Internal Affairs officers who may not be a part of HR.
Also, it makes the reveal of Terney's "loyalties" better because he was a good cop. Carter trusted him and so the audience trusted him in the beginning. That makes the betrayal that much sweeter when it occurs. "What? It's Terney? The guy we've been seeing almost from episode one?" It was like Szymanski, who we also saw almost from the start. He was accused of being corrupt, the audience was disappointed. Then he was cleared and we felt good only to see him killed while doing his job." It is because the show gets us to like the secondary characters that we get invested so that when there is a twist one way or the other, we're invested, just like the main characters. Too many shows don't have the vision or the budget to invest in secondary characters to this extent, but the story possibilities when they do, make it so much better for any show. Guest star of the week turns traitor is boring. Terney turning traitor is a gut punch that lives long after his death. Or, Hersh sacrificing himself at the end of season 3, even though we had seen him mostly as a "villain" through his run.

This episode is a work of art. I mean even despite Reese "only" being my 4th or 5th favorite character of the show, this episode still hits SO HARD each time. I agree that it feels a bit "random" and disconnected, but that's one of its strength in my opinion because you don't expect it.
The first genius foreshadowing that this scene in the car was not a flash-back but a vision, was that we did not get the usual Machine transition to a previous date. This of course capitalize on the fact that the show regularly has flash-back sequences so you don't realise the episode is THIS special at this point. Then of course there's John continuously increasing the heat of the car and complaining about the cold, but there was also the misdirection of the Elias subplot in the episode to make you surprised that it was a Reese-centric episode. Yet another hint is that in the vision/flash-back, Reese is the one who gives the picture, yet at the time Carter was alive, he didn't have said picture, she got it from Jessica's mom.
The music as well in this episode is fantastic. I mean... the music is ALWAYS on point in this show, after all we're talking about Ramin Djawadi (Game of Thrones, Westworld...). But here it goes above and beyond. The track when Reese reaches to his gun, the final long track for his talk with Carter... so emotional and moving.
It also ties in nicely the loose-end of Carter having Jessica and John's picture, as well as being an absolutely wonderful farewell to the actress and the character.
PS : People who call this episoded filler don't understand character-driven story-telling, this is one of the least filler episode of the entire show, and in my top 3 best episodes.

I heard one interpretation of this episode is that while John is hallucinating seeing Carter it’s really the Machine that is talking to him.

While this episode can seem a little out of place, it's one of my favorite in the show. In a lot of ways its the closure John really needed that he never got with the way Joss died. And the shots were just prefect. So many small details that you only notice on a rewatch. It's just great.

Bringing Carter back like this is something the show can only feasibly do once, and they used it masterfully.
I personally wouldn't call this episode filler. Reese got some closure with Carter, and that seems like important character development to me.

I loved this episode also. Fans were very sad to see Carter gone, so this felt a lot like a real way to say goodbye. Also, contextually, fans at this time did not know if there would BE a season 5, it hadn't been announced if there would be one and how long. So, again it felt like we were having a chance to say goodbye to a main character.
