    This was a realistic and detailed interaction between a pharmacist and a patient. so many aspects about her therapy may have been left out but most pharmacists need to appreciate that there is more to our profession than blindly handing out prescription medications and making a fortune. We have a very pivotal role when it comes to educating patients and listening to their questions privately.
    Pharmacists should take even more time to prepare for a medicine use review and encourage target patients i.e polypharmacy and those with respiratory conditions to name a few to habitually approach a pharmacist for medicine information atleast annually.

    I am a Pharmacist in the Philippines. And I want to achieve this kind of setting, wherein we were able to educate patients well and counsel them properly.
    Being a Pharmacist is not just about handling medicines, it is also about saving lives. Note that. There's more to it.

    Great job. am delighted. however, I think the pharmacist should have mentioned a potential contraindications such as not taking the sidernafil with any agent containing nitrate like Gleryl trinitrate or even alcohol.

    Wow! This changed my perspective for this job. I thought pharmacists just give you the medicine that you need directly. I never thought that they would also ask questions to the customer like a doctor. But can a pharmacist also diagnose a patient?

      Unless you have are a pharmacist who can prescribe medications then yes then can diagnose as long it is in their competency. However, most of times pharmacists can infer what the problem is but will refer patient to doctor to actually confirm the diagnosis

    you should have mentioned potential CYP3A4 interaction between diltiazem and sildenafil and if the doctor was aware of that prior to prescribing.

      interaction just if potent CYP3A4 inhibitors the dose of cialis may be decreased to 10 mg ..diltiazim is not a potent CYP3A4 inhibitor..so there is no problem

    Think she did perfectly well. Just one question . The doctor mentioned to the patient that she takes a high dose of diltiazem and digoxin and that they weren't working for her. So she should try something different. Should we confirm or not with her whether she needs to stop any of those medications and take this in place of those or in addition to it? And if so how to dose it?

    Great Job! Just one thing to mention: should the pharmacist ask the patient something about her social history? alcohol? caffeine, tobacco, diet or daily exercise?

      Yan Xia On the OSCE you only have 7 minutes to solve the case, so for the sake of time you should initially only ask questions that could affect your recommendation. The PEBC has published the actual grading sheet of cases on their website. You will notice that on this check list you are not required to ask about social or family history unless it is merited by the case and could affect your recommendations.

    In Brazil, pharmacists "only" exist to give license to open pharmacies...Here, employers are worried, not with patients, but in "how much money" they can earn from them...(Sorry for grammar mistakes)

      Mazen: she did not forget. She DELIBERATELY did not do so. On the PEBC web site are sample case videos and corresponding answer sheets. Offering a private counseling area is not something done in any of the PEBC videos and is not on the answer sheet.
      So doing so, won't result in a loss of score, but it also won't increase your score. But what it will do is waste about 10 to 15 of the 420 seconds you have to solve the case.
      So where did this idea that you need to offer a private counseling come from? In our experience, the only people who routinely do so are International Pharmacy Graduates because other people taught them to do so. It is a rumour that has been freely propagated. And sadly, some classes even teach students to say this.
      The only time we'd consider offering a private counseling area is when dealing with a sensitive subject (e.g. Erectile Dysfunction) or if you received some indication from the client that they are shy or embarrassed.

      the drug is sildenafil . even it's not for ED, people think the medication is for her husband.
      what about saying everything is discussed will be kept confidential?

      mazen a You do not need to say that everything discussed is private and confidential unless the patient brings up a sensitive matter and you want to provide sone assurance. In the PEBC website there is not even one instance of someone assuring confidentiality in any of the example case videos or answer sheets.
