  • Dr. Ebraheim’s educational animated video describes the difference between the conditions know as tennis elbow and golfer's elbow.
    Elbow problems typically occur where the tendon attaches to the bone. The problem occurs due to repetitive stress which causes interstitial tearing, inflammation and swelling.
    Tennis elbow/lateral epicondylitis
    Inflammation, soreness or pain on the lateral side of the elbow.
    Degeneration of the extensor carpi radialis brevis.
    Usually seen in patients who perform manual labor or spirts which require twisting and extension of the wrist against resistance.
    Differential diagnosis
    Radial tunnel syndrome: the pain is very similar to symptoms of tennis elbow. Pain usually radiates to the lateral epicondyle and radiates down the forearm.
    If the symptoms of tennis elbow are not going away with treatment, rule out the possibility of the radial tunnel.
    Golfer’s elbow/ medial epicondylitis
    Inflammation, soreness or pain on the medial side of the elbow.
    Pain due to repetitive overuse or overload activities which are aggravated by movement of the wrist.
    Ultrasound evaluation of tennis elbow
    •Elbow is placed into a semi-fixed position with the hand pronated.
    •Ultrasound is used to visualize and inject the tendon. The transducer is positioned parallel to the lateral epicondyle and the radial head.
    •The radial head is identified. The origin of the tendon is identified. The tendon is usually smooth and homogenous. The affected tendon may show tears, calcification or thickening.
    Treatment of tennis elbow:
    •Rest
    •Job modification
    •Nonsteroidal anti-inflammatory
    •Physiotherapy-eccentric exercises
    •Injection (can be blind or ultrasound-guided):
    1.steroids injection provides short-term relief.
    2.PRP(platelets with needle fenestration)
    Blind injection
    •The injection site is marked and disinfected.
    Ultrasound-guided injection
    •Needle fenestration
    •Injection of steroid or PRP.
    Surgery
    •When conservative treatment fails
    •Debridement of the involved tendon.
    •Surgery is usually successful in about 85% of patients
    •Avoid injury to the lateral collateral ligament

    I used to do a lot of weigh lifting in gym. In july 2020, I had an accident at my home that caused pain in the medial side of my elbow. Initially, I was not able to do flexion and pronation. I saw a doctor and after ruling out fracture, she recommended resting. As the rest did not completely restore my elbow, I saw her again, she further recommended resting, with the diagnosis of "soft tissue inflation", as she put it. Initially, upon further rest, things improved but in 2021, I was very busy working with computers and typing, which resulted in the worsening of things. In july 2021 I saw another doctor, who told me I have had golfer's elbow. However, MRI revealed that I had inflation at the lateral, and not medial side of my elbow.
    I have done 45 sessions of physiotherapy, 10 sessions of ergotherapy, and 6 sessions of acupuncture. Much of my hand force and range of motion is back, and I can do most things now, with the notable exception of typing and working with a computer and trying to move my fingers separately. It is also sometimes difficult for me to open a can with twist cap and write on a paper (which was initially very painful) . I do not have issues with tasks that need high force (lifting weights, pushing etc) but somehow typing is painful, which has rendered me unable to work (please note that I use an ergonomic split keyboard and a vertical mouse). My doctor told me that I might need to see a rheumatologist and/ or undergo surgery but I'm confused and afraid that the surgery might make me lose the sensitivity of my fingers.
    The exercised I do: stretches, wrist curls similar to what is shown in this video, eccentric biceps curls (with dumbbells and stretch band), resisted wrist twist
    (with dumbbells and stretch band) . I also used to have isometric exercises, but my physiotherapist told me that they might not be useful anymore and I stopped them. However, I recently have brought them back as I noticed my pain went up.
    In addition, I have now started doing some exercises for my triceps (eucentric with a resistance band and biceps curl with dumbells), as my physiotherapist said that there is too much of imbalance between my biceps and triceps after many months of working on my biceps.
    For all the weighted exercises, I started with 1.5 kg and am now at 3kg. I do not have pain with this weight.
    Furthermore, I use a Powerball for ~5 mins a day before my workouts.
    I have noticed that my pain immediately goes down when I do the exercises, especially the ones involving heavier weights and resistance. I also apply a warm press and a message gun after my exercises, which also seem to help a bit.
    Touching and pressing on the interior side of the elbow was initially very painful, but now I almost do not feel any pain unless a major force is applied. In addition, I used to have numbness in this area, which is absent now unless I do a lot of typing.
    Do you have any recommendations? Is there anything you think I should do? Also @everyone else

    Excellent video. Your explanation is superb. In desperation,waking up at 4 am,in pain,I stumbled across your video,and,it makes all the difference when one can see exactly what the problem is,and,fully understand what caused it. Not golf,or,tennis,but.....stacking firewood blocks,and,doing a bit too much all at once. Now I suffer the consequences,but,watching your video has made me stop,and,take stock. Gawd,the things we do to ourselves. I'm an idiot.

    I had a botox injection on the left side a couple of years ago. Amazing benefit with pain relief in three or four days and complete resolution. Of course, I had weakness of my fifth and fourth finger for too long - 3 months or so - but it definitely helped. The description of the injection site is in the Canadian Medical Assoc Journal around 2009.

    Sports massage therapy is the best treatment possible for these type of injuries. Prior to injection and surgery. Therapy can not just rehabilitate but restores strength and a good therapist can give recommended ways to stop it occuring again.
    Rest, ice, reduce repetitive motions, take an anti inflammatory ( short term) relief. If you have received a blow to the elbow it maybe a burst or inflammatory bursa. Ps I am a Remedial and sports massage therapist. Good luck

      I have had tennis elbow for a couple of year, had 3 steroid injections but its just a quick fix. So painful so now iv been referred to a physio. Hope it helps. Thanks for the imformation

    Excellent information. I had an ultrasound guided injection for my golfer's elbow. Feels much better; now I intend to rest it for a number of weeks, at least three, and then begin some more strengthening exercises before I go back to my full exercise routine.

    I have both. I was a nurse for 10 years. I've had therapy and i received a steroid injection. I was told not to get them too often because they could make the problem worse. I have to have surgery. If I can give any advice to anyone, please don't hesitate to see your doctor with this because it's painful no matter what you do if you don't get some sort of treatment. You don't want to HAVE to have surgery.....try to avoid it if you can. Great video.

    Before I had shoulder surgery, I was having pain in my wrist and elbow, after surgery I noticed on the lateral side of my elbow was swollen and puffy like there was fluid build up for a few weeks. It is over two years now and sometimes the pain is unbearable. The doctor said it's tennis elbow, but nothing is been done. I hurt myself at work. What's your advise. By the way, your video is very clear and helpful. Thank you.

    is there an error in the video when he ddx'd tennis elbow with radial tunnel syndrome? lateral epicondylitis and radial tunnel syndrome are two completely different pathologies, but he said if tx of tennis elbow doesn't improve, rule out radial tunnel too -- that doesn't make sense. wouldn't you possibly begin to treat radial tunnel syndrome if tx of tennis elbow doesn't improve? am i understanding this video correctly?

    I have both on my left elbow and just golfers elbow on my right .Just been managing to get through day by day at work with the aid of pain killers and anti inflammatory medication.Until you get it you never know the condition is hell on earth.

    I never hurt myself but my hand hurts just at the elbow joint. Its like the little bone at the elbow hurts. The pain is at night. During the day the hand is heavy and it feel like blood is not flowing

    Cortisone injection should be avoided as the condition is usually not inflammatory but rather a stress reaction ie. a result of repeated microtraumas. Cortisone has a short-term effect, but recent studies suggest that cortisone is prognostically negative in the long run.

    it just gets worse unless you stop using your arm and put a good wrist brace on and for the first few weeks a sling. I'm in week 4...ridiculous slow heal time for this injury...

    How long does it usually take to heal? A heavy blow when soccer goal keeping gave me golfers elbow. Been 6 weeks so far but still some pain at certain times of the day when bending the elbow especially in the mornings.

      Ice it at these times of day when it gets irritated. Stop it getting irritated. Buy a epicondyl strap. Rub the area lightly as it cancels the pain.

    I would definitely see a good massage therapist.. The reason there is inflammation on the tendons is because the muscles are pulling on the tendons

    dr. i lost both legs as a infant. im now 44. i have had 14 elbow aurgeries from 4 ulnar nerve trans. 8 ir more scopes and i keep tearing tendons now. i have had 3 surgeries on elbow since last april. i tore it again and now face another operation. do you have any thoughts on how to keep from retearing everything. ladt timee it was flexor tendon mass . they think its even going again. i need help sir. last surgery was done by dr morrey at mayo. cant afford to go back so osf in peoria il is doing next surgery on friday. dr jeff garst is surgeon.

    Thank you for your interesting video sir.
    I am a menigomyelocele and use elbow crutches to mobilise. For some time now I have had left lateral pain radiating from elbow to forarm and obviously it is not possible to rest the elbow so I am considering using axilla crutches at least for a while.

      There are many factors in ways to treat tennis elbow. One plan I found that succeeds in merging these is the Freds Elbow Helper (check it out on google) definately the most helpful guide that I've heard of. Check out all the extraordinary info .

    Very informative video. However, why wouldn't you recommend massage therapy as a form of treatment? There is a lot a good therapist can do to alleviate symptoms of both tennis and gofers elbow.

    Could tennis elbow be the root cause of my severely itchy anus? I wake up in the mornings, and my fingers are heavily soiled, brown, and smell of feces. Can you treat this with an adjustment, or some shoe inserts? Thanks.

      It's not, I have Neuro deficits from an explosion and shrapnel striking my head. I used to be able to sleep ok, but now I have to stick my finger up my anus to fall asleep.

It is a smashing one of a kind guide for learning how to cure tennis elbow without the headache.
