I think you hit the nail on the head with the open world design of Elden ring: there's a LOT of just empty space in this game. Sure, it looks nice when you first step out into a world like this thinking "wow, there's a lot to explore". But once you realize it's literally just space you have to traverse it quickly loses its magic. The this with the round table hold is really weird. Milena speaks as if you had accomplished some great feat when you really haven't. I'm rather sure this is supposed to happen after you've beaten storm veil castle. Both Milenas Words and the arrival of that other spirit attuned lady at the hold point to this. For her, getting out of storm veil only really makes sense after Godrick dies. She's seeking purpose, but up until Godrick is defeated by the player, her purpose was to be grafted, only she lacked the courage. But since it's possible to just walk past the castle, the round table invitation comes across rather arbitrary and random. You probably found out at this point already, but the white fog walls can be dispelled, usually by using a stone sword key at the imp statue in front of the fog gate. You just missed it in the basement there.
