So, to get a B (350-440) grade we MUST need to get in Purpose 2 and 5 in all other criteria
ISBAR:
I: Introduce & Identity
S: Situation

Now the BIG question is:
How to choose RELEVANT case notes?
Ask yourself two questions:
1. Is this case note relevant to the current situation? Yes/No
2. Will this case note affect how the reader continues care for the patient? Yes/No
If the answer to both questions - No = NOT RELEVANT
If the answer to either question -Yes = RELEVANT
NB:
So we must include in our letter the RELEVANT case notes otherwise we will penalize.
And
We must exclude in our letter the IRRELEVANT case notes otherwise we will penalize.
What to do with the aspects of past medical history that are either normal or not very important?
For example:
No smoker, no known allergies, no further hair loss, no current medications, no significant medical history, no surgical history, no reported drug use, no pregnancies
So how we can summarize these notes, we can simply write the patient's medical history is unremarkable
For Example: if the case notes are like this:
increase smoking, smokes 30/day
Increase alcohol on weekends. Drinks 3 wine glasses at night
So we can write:
She is a regular smoker (30 cigarettes/day) and drinks moderately on weekends.
Introduction: IPC2P
I: Initiation (Referral letter): I am writing to refer
P: Details of the patient: Ms Peterson, a 56-year-old single lady
C2: Chief complaint/Main Issue: who was diagnosed with………
P: Purpose: She requires your advice and management
I am writing to refer Ms Roberts, a 56-year-old single lady who was diagnosed with Chlamydia trachomatis. She requires your advice and management.
Sentence Structure:
Simple Sentence:
She is a regular smoker
She drinks moderately on weekends.
Compound sentence:
She is a regular smoker and drinks moderately on weekends.
• Example from case note:
Weight: 86kg, height: 157cm
Peterson concerned about weight gain of 8kg over last 12 months
Sentence:
She has gained 8kg over the past 12 months and currently weighs 86kg.
Synonymous languages:
Post-operative=Recovering from/after
Further care/follow-up care
Palliative care/hospice care

Example:

Present Perfect Progressive: Repeated action that started in the past and continues to the present.

Example:
He has been suffering from migraines since 2010
He has been smoking one pack of cigarettes per day for the past ten years.
They have been trying to conceive for the past 12 months.
Use of the past tense:
1. When describing an event in the past which is resolved
Ex:
Mary gave birth five weeks ago.
(the baby has arrived and there have been no ante-natal complications from the birth.)
2. With diagnosed
Mary was diagnosed with pneumonia and treated with antibiotics prior to discharge last week. (pneumonia is now resolved)
3. When the situation is ongoing but is stable (diabetes that is well-managed with medication.)
Mary was diagnosed with diabetes in 2008, for which she takes regular medication.
(although this patient still has diabetes, the diagnosis is considered a complete event now that the condition is stabilized.)

1. When the condition is unstable
Ex: Mary has been diagnosed with asthma and prescribed an inhaler. She will return to the outpatient clinic next month for a review of the inhaler’s effectiveness.
(asthma treatment is still being finished. If the inhaler doesn’t prove effective, then a different treatment will be considered.)
2. When the condition will be news to the reader
Ex:
Mary was admitted to hospital 3 days ago. She has been diagnosed with unstable angina and requires your follow-up care.
(the diagnosis is news to the reader and how they will be involved going forward.)
