- EMS and first responding is the best job I have done yet. I get paid to eat, be social, workout(sometimes) and serve my community helping those in need. 48 hours on and 96 hours off shifts hard but when surrounded by good people they make it better.
HIPPA prevents me from filming patient care or much more and I was unable to get our tones on video. But this gives you an idea of what we do on our shifts at our two man station. Some of our other stations with this department house with the volunteer firefighters as well. It’s not all the same everywhere but you’ll get the idea!

Local EMS service here runs 48hr shifts and they have had a bunch of crashes due to employee fatigue. Unless they give you a solid 4-6hrs a day to rest, it’s a very dangerous shift. The 4 days off are great though.
Been on this shift for several years and I know of multiple other departments that also run it. Have yet to hear of a fatigue related wreck.

It’s give and take. Some days I’ve had 48s that have been just nonstop and I felt like I was on auto pilot and then there have been shift where I only had two calls in 48 hours and so you get what you get

I pass my EMT National Registry in one try and got a job as an EMT a week later. I'm still in training. I love it, can't wait for the paramedic course. This video is awesome

We work 12s and 24s in my city. We get run to the ground. 8-10 calls a day on 12s. Weekends usually just 3 or 4 calls. Everytime I hear someone working a 48 I just imagine they get ran none stop. 24s are alright. Stay safe and love my fellow EMS personnel.

I’m shocked they would allow more than a 24 tour. That has disaster waiting to happen all over it. Even with time to sleep... it’s never a guarantee that u can get solid sleep & creates a potential for mistakes to be made. Just sayin but, God bless & stay safe. I hope you pass your medic!
If it gets too bad or one or two trucks are running many calls back to back and there is no time for sleep our supervisors are great at having our back and giving us a few hours at being last up in rotation so that we can catch some ZZZZZs

The longest I've ever worked straight through was 98 hours. Granted I had rest times it wasn't non stop. I did eat and shower etc but I was getting paid to do that. I could have gotten a call while in the shower. I had people that would run a call for so I could shower. I only worked those hours due to extreme low staffing and in a disaster situation. The low staffing was from the majority of crews voluntarily went to go help out on hurricane Katrina. The other factor was because of hurricane Katrina my area had major flooding so we literally were evacuating entire towns that included a hospital and several long term care centers that were flooded out.
We’re allowed to work 96 with a minimum of 12 off required after.
I'm a EMT-B I'm here because I like to see how other EMS agencies do things, I know some of our locals one's here have life packs, while others including ourselves have the zoll EKG's. I tend to stick to 12-24hr shifts, but have done 36-48hrs. 12-24hr shifts work best for me. I will say they had me and my partner cover for another EMS agency and their tones are different from ours and I could not understand their dispatchers neither lol, Im glad I work for the agency I work for I know our tones and can understand our dispatchers better, we transport to 6 different hospitals. But mostly 4 90% of the time. Great vlog.

Great video. I was always curious what the machine was that does chest compressions but now I know it is the Lucas Device. I saw EMS using it on a patient they were carrying out of Walmart one time. Nice EMS rig and thank you for all you do and being there when we need you. 🚑
I love transfers, now I am going on my 3rd day of a 24hr shift.
I just started at a transfer company two weeks ago. I will be doing that along with my 911 position. I am excited to see how it goes and what i can learn from it!

@@TheFultoneers I am EMT-B. I was first doing ALS for a year and then been BLS for 3 years. ALS service I worked for did not treat me well so I left. Now I work for a great BLS service. For a large healthcare provider out of Sioux Falls. I am trying to finish my paramedic school I noticed you have the same textbook, the JB Learning Book Nancy Caroline's 7th edition. I am also a Firefighter volunteer. As an EMT I get paid on call pay and then full pay when a call goes out 911 or transfers. average 13-15 dollars and hour for EMT-B. Paramedic is 25 dollars in MN. I am not in it for the money I do it to serve my community and have passion to help. Best of luck in Paramedic School.

I actually finished school and have been taking paramedic level patients for three tours now.
The transfer company I had started at shut down the location I was in. But it was for the better. I did not enjoy the people and they had no heart in their work. It was sad and I struggled with even wanting to go in for my shifts there.

48s aren’t bad if your in a rural areas, but where I’m from just outside the big apple if you work anything more than a 16 hour tour your shot by the time your relief comes in.
For sure. I know 48 seems daunting but for us and our company the rough days don't out way the "slower" days. Rarely do we not got some kind of downtime.

@@TheFultoneers oh fun. Anything paid near here is a single commerical company that is 911 and does transfers. Busy place to work though. Eveything else is 30 minutes to an hour away.
How do private/commercial and volunteer services work? If a 911 call comes in do all three get despatched? Sorry if this sounds like an odd question. I'm in Australia where each state has only one (state government run) ambulance service. We can have two stations 1700 miles apart and both have staff from the same service (same equipment, drugs and training). We've got 4,000 staff and 290 stations.

I’m hoping going to be taking classes to get my EMT certification in the upcoming spring semester. Do you have any advice? And what’s your favorite and least favorite thing about the job? My biggest thing is the possibility of losing a patient and not knowing how to deal with that, do they go over it in the courses?
Yes they do go over that in the course
There is a few chapters actually going over stress management, and PTSD. They want you to be the most prepared for the job you are going to take on, and don't be discouraged if the job isn't for you, it's not for everyone.
Easiest thing to remember, it’s not your emergency.
It is hard loosing a patient young or old. You don't get used to it but you learn to put it in a special place in the back of your mind. You have to do your best and remember your skills and be sure you did what you could. But don't dwell forever on it. Give yourself a few hours or maybe a couple days ,everyone is different, talk to your partner or even you upper staff. You are surrounded by a huge brotherhood in EMS/FIRE/PD. These people around you know how you are feeling because odds are they have been there too. Don't be afraid to ask for help or to just talk through a call. You will most likely cover this in the first few weeks of class.
Study! If you want to get a head start, see what book you will need for the class and start reading it. It is a lot of information, especially if you are doing a hybrid class.
Least favorite: running a bad call involving a child. Running a call with an abused or rape patient. Watching that DNR patient die because their last wish is for you to let them die. These are all tough and you will learn to deal with them. I am not saying it gets easier, just learning to deal with it can get easier.
Then you have the favorite things. Some of those are seeing the relief and greatfullness from patients that you have helped. The hugs and smiles they give you. There are times that you just think "this is what I am supposed to do. This is why I do this."
Your instructor/s does go over this in class. Your book goes over it a little, too. Find yourself someone that has been doing it for a while, and ask them for their advice and how they handle bad calls. Finding a hobby if you don't already have one is a good way to cope with the job. Exercise is a great stress reliever!

Hey I'm over here in Connecticut, starting my EMT cert. Luckily we get high wages in these states.
Getting a job is not too difficult where I am. I would recommend that when you do your clinicals try out many different locations. This helped me in seeing the call volumes and what that area/ service mostly received calls for. Also, you will get a glimpse at the work environment and whom your coworkers may be.
Just completed by emt course, just waiting for registry exams in two weeks!!

I work 24 hr shifts sometimes I will pull a 48, however, our call load is not heavy most of the time.
In Jan 2021 our truck ran 137 calls

I have been in EMS for 26 years. Four years as a basic EMT and twenty to years as a paramedic. I love what I do. The pay wasn't always good but do very well. I make well over a hundred grand a year. My advice to you is to work for a private services for a bit and get some experience. Then get on with a municipality. We in EMS live selfless lives. We give time with family, we work on holidays. We work on the hottest days and in blizzard conditions. We get little sleep and eat shitty food. We put our lives on the line so others may live. I'll do it again over and over in a heartbeat.

This guy I work with, who was working in EMS for about 13 years, told me that the Basic EMTs are the grunts of EMS. Basically, most of them work for a private ambulance company, transporting patients from their homes to the hospital, nursing homes to the hospital, etc; Then, even if they are hired in with 911 providers, the basics are the ones driving the ambulance and then cleaning out the back afterwards. How true is this for a Basic level EMT?

He is not completely wrong. Here in Texas there are select few things basics have in their scope of practice. Your items in your toolbox are not as extensive as a paramedic.
In the transfer setting your calls involve getting a patient from point A to B and unless a critical patient, which would likely be a paramedic call, is on your stretcher you don’t have much you can do. Here you learn from not the care but reading the paperwork provided with the patient about what is wrong, why and what has been done to treat them. You have to be proactive or you fall into the rut of the “typical” transfer medic.
In the 911 setting you can still do some of the above depending on the patient. If you are on a basic/paramedic truck and have a good partner you learn. You may still be the one driving depending on your protocols and if you can clear patients to pass down ect but you can talk things through with your paramedic.
I’m working on several videos at the moment. One of which is the levels of certification and what they can do!
We have a scope of practice in place for our EMTs at my department. All of our units are ALS and have 1 medic and 1 EMT. But for all calls, 911 or interfacility (when have a small hospital we transport out of) if the pt. falls under the EMTs scope they can attend the pt. even on 911 calls. So it's really good for them and makes them much stronger EMTs. But every department and company is different so really it's up to them and their medical director.

48 hours. What's your call volume. No spare ambo to use when regular ambo overshift? I hope you get some solid down time. EMS accidents mostly caused by fatigue....often fatal.
Thanks for your comment. Our call volume here varies from 0-1 911 call to 8-10 for a 24 hour time period. Our supervisors are awesome at giving us down time if we have been running hard. If needed we have personal that can get on a spare unit to assist.

Here you have to be an RN/BSN as well as Paramedic certified. That's about 4-6 years education plus you need a couple years emergency room work experience.
Here in Australia it takes a few more years to be an FP. Three years uni plus one year intern just to be a qualified registered paramedic. Then two years experience before trying out for critical care paramedic (CCP course takes 1 year), then minimum two years as a CCP to try for a flight paramedic spot. That's 9 years and it's extremely competitive.

I work a 48 on 96 off schedule so every week I get about 4 days off. The first day is usually spent recovering especially if we had a busy shift with no sleep and the others are spent at my other job or with the family.
You get whatever partner you get put with. Male/male. Male/female. Female/Female. Just depends on where they need a person. Depending on the service you work for you can be on a basic/basic truck a basic/paramedic truck or even a paramedic/paramedic truck.

Is it just a room of beds, or do you guys get your own rooms and whatever?

We have two all the stations. One for the in-charge and one for the attendant. Each room at my station has a TV, bed, desk and side table. Check out my Instagram and I have some pictures from when I started painting our station back in February. I plan on doing an updated post and station tour!

this comment literally spoke to me !! I’ve been dwelling on that for so long bc I’m in a very similar shape as you but I’m just going to go for it in October; I say you go for it too because your mind and motivation is what should be moving you !
The one we have in our service is not only a paramedic but she is also a firefighter. She gets a lot of crap for her size but will kick but on both fire and EMS scenes. She worked her butt off the get things done and without help from others to show she can hold her own. She even kept her own step stool on the fire truck to get what she needed. If you love it and have the passion and drive don’t let your size stop you! Adapt and overcome 😃

We do a split 3/4 at my department - 24 on, 24 off, 24 on, 24 off, 24 on, 96 off.
I’m going to get my emt certificate this fall and hopefully pick up a job. One question tho...does it ever get boring when your not on calls? Or is there usually a lot to do? Ok it depends on what kinda place you work at but still lol
OMG yes! One day we watched 16.. yes.. 1. 6. hours of Walking Dead. surprisingly in our area when it rains everyone just kinda stays in so our call volume goes down. But there are also days when for 16 hours we haven't made it back to the station to even pee. There is no way of really knowing how the shift will go. You just start shift and hope for the best!

I have always wanted to chase anything that can turn tornadic. Any chaser or people in the path of those storms will probably need medical attention in one way or another. Would love to be that person!

So maybe its possible to use caffeine only during those shifts to keep you awake then go off it when you aren't working?
Yes. I only drink water at home. Occasionally I will make a cup of coffee but I really only drink caffeine and coffee on shift or long road trips
How many calls did you do in your 48 hours ?!? In 24 hours in my city we average anywhere from 10-30 calls (30 being extreme but it has happened multiple times)
Our calls last about 2-3 hours each.
In one 48 at this station we can run anywhere from 5-18 including refusals and transports. This station is the busiest of our 5 stations.
Ok so after a 48hr shift it will be followed by how many hours off?
For this schedule we work 48 on and 96 off unless you pick up overtime during those off days!

Study hard. Take it upon yourself to look up things you don’t understand and do not be afraid to ask questions. If you are not sure how to use equipment or what to do ASK there is no fault in not knowing and asking rather than trying to wing it and possibly hurting a patient or yourself.

Paramedic school was difficult. It isn't for someone who does not have the drive and passion to keep moving forward.

Absolutely , I love my job, the people I work with and even though it is stressful at times it can also be rather rewarding! See if you can do some ride outs with a local department and find out for sure if it is your cup of tea before signing up for classes!

Depends on the state and agency. You should have some form of EVO training either with fire, law enforcement, or ems. We had times where we were the only tech's in the back with a critical patient and we needed someone else to drive, usually that's law enforcement or the fire department for us. I hope that answers some questions.

Yes, some transfer companies have wheelchair vans which are transporting people to dialysis or doctors appointments. These people are usually just out of HS and working on their EMT or some kind of medical certification!
Yes. My dream company, MCHD (Montgomery County Hospital District), requires you to be a paramedic to do all of the "fun" stuff. However, you can drive as an EMT.

What is the plug for that plugs in to the truck got ?
That is our shoreline. It is to provide power to the truck. That way we have AC and our cooler and plug in the back of the box all work while we are providing patient care. :-)
How many days are you require to work for a week? Do you get days off?
Every location is different. We are a 48 hours on/ 96 hours off schedule.The thing is like about this specific schedule is being full time every pay check I acquire PTO and if I take off one 48 hour shift iI get 10 days off. That being said we also have part-time people at our department that pick up when people take off or we can trade with other shifts.
When u have doa do you guys got call in ask for time of death or do u guys do it ourselves I debating on going to school for EMT or Rn
ChelsyProudMommyOFBabyGirl Shepherd Thank you for your comment. It depends on your protocols and if there are visible signs of death such as lividity or rigor mortis. These are reasons not to start CPR. For our county the paramedic will determine the decision and we have to call a JP and turn the scene over to the sheriffs department. In a hospital setting a doctor will be the one to determine the final choice and call time of death.

ChelsyProudMommyOFBabyGirl Shepherd My goal is to go to nursing school. In some places there are paramedic to RN programs. Starting out as an EMT basic is a great way to start. You decide if you like the patient care and you learn to make patient decisions on your own. I recommend starting out as an EMT basic at the least and/or paramedic before beginning nursing school

I’m 16 RN and I really want to be a paramedic when I get old enough but I do have a question 🙂 is it hard to complete the EMT training?? Coz I’m SOOO scared that if I go to school for a paramedic I won’t pass lol
So many people have this fear. EMT I felt was more common sense than anything. If the patient is not breathing breath for them, if they are bleeding then stop it and so on. Paramedic was more difficult because you go more in depth of body systems, medications and the more things that are your responsibility. I will say don't start transfer and after you get your EMT done start somewhere working as an EMT to get that patient contacts in and feel more comfortable and understand the process when you more on to paramedic! Tips for emt school?
Study hard. Books help but the real learning is after you pass national registry and get into the field. Listen to what your preceptors do and watch what they do with your patients after you get them to the hospital!
Whats the deal with the clinicals after shift? Is it paid? What purpose does it serve exactly
Ryan, I was in the process of finishing up my Paramedic school so when I would get off shift from my job as a Basic, I would head to a Paramedic school clinical to collect my hours for completion of the course. Unfortunately, no they were not paid but are required to receive my patch.
@@TheFultoneers What's involved in training to be a paramedic in the US? Here in Australia you do 3 years of university, then 1 year as an on road intern. After that you're a qualified registered paramedic. We don't have volunteer or BLS level here; all ambulances are crewed by 2 ALS paramedics or sometimes a critical care paramedic and an ALS paramedic.
Holy crap, 48 hr shift isn't even legal in California anymore, hope they give you a week off after.
4 days. Not too terrible. With our schedule I enjoy that I can take off one tour and get ten days off!
Who drives the EMT or paramedic?
Hi Alexa! Great question. It depends on the type of calls you are getting and what level can ride the call into the hospital. If it’s a paramedic call the basic drives and vise versa. When two partners of the same level work together they will typically switch out calls and driving every call or every few calls depending on their own arrangements.

You respond to the call soap and all! Any field of vision needs to be dried off such as the back up mirrors but besides that nothing changes. Same thing if you get a call while you are in the shower... I have gone to a call with dripping hair before haha

disastrix47 rule of thumb at my station is learn how to do anything and everything in 5 minutes or less so you never have to worry about that 😂😂😂

We managed to heat it and eat and as soon as we had just about finished we got a call and ran 4-5 back to back. At least we ran them with full happy bellies!
We have a 2 minute chute time for all calls, this includes response time, so a two minute chute time = a two minute shit time. Take your showers really early, 0500-0530. Slowest time for us. Our system is a mess, politics, and instead of driving to calls we spend the bulk of our shift driving to post for other providers who don’t want to post or cover their areas but expect others to do so while they run transfers and calls in their areas. We run a 2 on 4 off schedule. I’m currently on the tail end, 16 hrs left, on my shift. So..........yeaaaaa!

Try going to your local community college they may have emt programs there to get certified
community colleges and if not, it’s very simple(this is what I did): just search up “emt programs in rwanda” or “emt programs near me”

Why plug truck in
Great question! We plug the truck in with a shoreline. It charged the generator and keeps the air on while we are at the station. It's just like how you would plug up an RV once at a camp site.
Medical equipment requires power, heating or cooling and we don't want the car battery to go low; That's why we plug them in at the station
This seems criminal to me. No one should expect you to work so long. People need lives and good rest. It doesnt make since to me. You are essentially saving people how do they expect you to do that when you're fatigued?

So I drink tons of water on shift! As a result I pee SO much. I don't like using the restrooms at the ER but sometimes it is needed. I try to wait till we get back to the station if I can but really you just go when and where you can!
I've literally cut back my water intake (i know i know...shame! Shame!) Because we post for calls...sooooo bathrooms are scarce between post moves and calls..., the guys can just pee in a bush (on nights).....lol
