  • Will your baldness gene ever switch off? When will the baldness you are experiencing slow down or stop? How are age and male pattern baldness intertwined? Does the devastating effect of baldness pass off like a storm or it continues forever to surreptitiously ravage a man’s vanity & confidence? Let’s find out!
    The irony of male pattern baldness is that it strikes a blow to one’s self-esteem and youthfulness (S.E.Y.) at an age when you need hair the most! The age you wish to look and feel your best.
    It’s all about your genes you inherit or your DNA as some refer to it.
    And there is no one gene. In those with androgenic alopecia, several genes interact with one another to determine the destiny of your crowning glory making your follicles susceptible to androgens.
    & baldness is not an all or none phenomenon. Like Blue Eyes.
    Male pattern baldness is a quantitative trait.
    So the likelihood of your going bald, the age of onset of baldness, the grade of baldness, the speed of progression and the timeline till the gene switches off or goes to sleep or hibernate is determined by the interplay and level of penetration of various genes we inherit from our forebears.
    & the longer you have been thinning, the more hair are going to be irreversibly lost.
    Baldness at the commencement of middle age slows down or stops completely due to the ebbing of the androgenic tide. This is what some people refer to as stabilization of androgenic alopecia.
    However,
    androgenic alopecia in the vast majority never switches off in the true sense! It only sleeps or hibernates- lies dormant.
    Hair loss suddenly starts to accelerate as we reach the end of middle age around 55. Both in those who thought that the gene had switched off and in those who continued to thin. This is the final strike- the Final Assault.
    This is because the body’s cellular pathways change as we age. The body suddenly starts to produce more 5AR that converts more of testosterone to DHT. This single phenomenon also called hair loss of aging causes both prostate to enlarge and hair to fall off.
    So what happens is that those who are bald become ‘really bald’!
    There is also bad news for those who never experienced baldness before. They also suddenly start to lose their enviable mop.
    The whole condition is further aggravated by sun damage to aging scalp skin since it is devoid of hair. So it becomes increasingly susceptible to UVA radiation. This destroys the superficial stem cells of the follicles.
    So hair loss never switches off.
    It may hibernate for a couple of decades but never really can be wished away for good.
    Either we treat it if we are a candidate for it medical or surgical hair restoration.
    Or we accept it.
    If both treatment and acceptance do not happen. let us deny it outright!

    Okay Dr... I will start using umbrella b4 stepping out. Apart from this, I'm using minoxidil n finasteride accomplished with a healthy diet. No blue light exposure..No smoking ..No alcohol. B4 going to sleep, I keep my phone away. This much I'm doing for my hair. If still I loose my hair in future, then last but not the least, I will accept baldness as my destiny.

    Sir my hair have become completely thin by age 21, starting hairloss from age 16. I had severe acne too in those times, face and back.but after 21 I had very low hairloss and no acne,seems like my androgens has become stable.
    I used minoxidil and my hair is getting thicker ,I don't have a massive shedding too. Does that mean my baldness gene has switched off for now.

    There should finasteride injection in the scalp like PRP procedure to stop hair loss at young age or maybe some new treatment where someone enjoy full hair.

      No cure to baldness is possible. Genetic problems have no cures. Genetic engineering is a long way in coming.

    I've a query ! I'm on fin for a year and I'm having wet dreams twice , sometimes thrice a week . I was having wet dreams once a month before I started fin . ( I don't masturbate ) . Is this a side effect or is it normal ? I'm not having any other side effects.

    Very informative. I am 60 now and notice some less hair volume... is there anything I can do to slow this down? I take Biotin supplements and eat good protein

      Volume reduces with age. If you have AGA medications will help to decrease DHT. Otherwise you could consider Minoxidil 5% application at night.

    I started balding at 18 and ended up with around Norwood 5 at the age of 24, I recently did a hair transplant 3 months ago and im growing some hair, but I need another transplant for the back of my head.
    I'm honestly pretty afraid to take meds, they don't have finasteride where I live either, only propecia.. I heard it can give nasty side effects, or even ones that are irreversible even after stopping the drug. I also worry about taking it for many years and god forbid get some medical issue due to long term intake. So far, I haven't taken any medication but my hair is also growing fine so far.

    Hello. When going to a dermatologist/tricologist. Can we have a shaved head ( 0 guard ) and do the trichoscopy examination still? Can the 50x camera magnitification see the difference beetween normal hairs and miniaturized ones? Or better to have a longer hair lenght to exam be more precise? Thank you.

      He will not be able to garner much information unless hairs are atleast 5-7 days unshaven.

      How has the growth been? If the hairs have grown thick, you can start taking it on alternate days for 3 months and then 0.5 mg on alternate days.

      Depends upon several factors. This needs an in person consultation to decide the dose. However 60 year old men with more or less stable hair loss can benefit by microdosing on Finasteride.

      People do not mind being on it when suggested by a Urologist! Most people have prostate issues at this age and are on Finasteride.

    Doc, can one get a hair transplant of strong hairline and maintain the frontal hairline without finasteride or minoxidil while he uses partial hairsystem/piece to cover up the crown and middle scalp

      That is a commonly done procedure for hairline. The donor area needs to be permanent and baldness gene needs to have switched off if you intend not to take Finasteride.

    If we could find a way to genetically alter the pathways for pattern hairloss both in men and women, even remove the genetical components that is driving hairloss, do it in young men and women, so they're no longer a carrier of these genes, so by the time they bring offspring it will never ever be a concern, artificially weeding out a genetic defect that mother nature didn't, and yes i consider pattern hairloss an actual defect, it has no benefit at all, i think it's just one of those faults that has haunted us for a very very long time, i say let's get rid of it forever.

    Do you suggest your patients to wash hair everyday after HT (talking not about immediate post HT but post 1-2 months since HT) ? 2nd question is does washing hair everyday affect the new transplanted hair in any bad way?

      Twie a day for 2 weeks ith medicated solution that is anti bacterial. Thereafter when crusts have separated,I ask them to wash daily but with shampoo on alternate days.

      @@hairtransplantinindia and that shampoo on alternate days is antifadnruff shampoo containing ketoconazol or coal tar ? Or either is fine ? So in other words, washing hair with anti dandruff shampoo will NOT cause any bad effects on the New hairs that are yet to be grown, correct ?

    When can we start combing the scalp using a comb after HT? Is it allowed ? I wanted to clean the scalp using that but worried if it might hurt the growing new transplanted hairs.

      Are the native hair not trimmed out completely? If not, you can use it after one week but it should not touch the recipient scap- only the hair.

      AA is not androgenetic alopecia. Finasteride will not work.

    Doctor thank you very much for your work and support. Doctor I have a question. Does finasteride acts differently based on height and weight. I mean if a person is short 0.5 mg of finasteride is more than enough to see visible results and if a person is tall even 1 mg of finasteride is not enough. Please help me with this query.       The effective dose that is proven is 0.8 mg a day. 1 mg works for all weights.

      A hair transplant is not a DHT blocker. It just fills in where DHT has taken away the hair and this process will continue.

    Hello Dr. My brother experienced slow balding in his late thirties and throughout his forties. It was there, but not very noticeable unless you looked hard. Since 5O, the process has accelerated a lot, and he is now Norwood stage 5 at 53. Is it likely he will now go to stage 6 or 7?

      The pattern that is encrypted in his geentics will unfold to the final grade destined. But he can take control.

    I was balding thinning at the temples at 16 and now I’m 23 with a Norwood 2 with frontal thinning and mild crown thinning. What stage of baldness do you expect me to be by 30? Do you consider my baldness to be aggressive?

      From what you say, it seeems you are heading towards minimum grade 5. However, grades of baldness cannot be accurately predicted. It is seen that when baldness happens at this age, it usually is aggressive.

    Hi Dr is it normal lose 20 to 25 hairs while shampooing? I have male pattern baldness and I’m consuming finasteride for almost 1 year.

      One loses 100 hairs a day and grow a similar number. But when we have the baldness gene the hairs that grow back are lesser in number and when the deficit becomes stark, baldness begins to appear. You are doing fine. Do not stop medication.

      PRP is not a DHT blocker. It benefits but just in boosting hair growth transiently. Not a replaceent for Finasteride. I do not recommend it.

    Dr diep said balding will never stop, because when you become old you will still losing hair. Because we all become older and hair become very thin. This has too with genes what we also have. Mosts guys wearing a wig that is called ' a hair system' Even hitrik roshan is bald and wearing a hair system, because his father was bald too and his uncle too

    Sir, please share some information about redensyl . I'm using minimalist 18% hair actives which has 3% redensyl and other 4 actives. I'm using it for a week now . should I continue using it? .what is your take on redensyl and minimalist serum?       I am not a fan of medications that come and go and medicines that the medical community has not much experience with. I follow only proven methodologies and do not experiment since the patient has a small window for using the right medicines. I will not guide him to medications that may not help.

    So Science did not know as much as it thought it knew when it came to advertising the benefits of a hair transplant.
    We used to hear that the donor area was the genetically safe zone where man would always have hair. The term "permanent" hair was also used. But haven't you said before that hair in the donor area can miniaturize?
    So, which is it?

      Nothing in the human body is permanent. Clinics do use the word 'permanent' while advertising. However the correct phrase would be 'relatively permanent'. Hair in the donor too miniaturises though to a much lesser degree.

    How many cases have you seen where a patient went to Norwood 2 at 20 yrs old, Mabey even Norwood 2.5, and the balding switched off? Have you seen that before? Like Christopher Nolan's hairline, or Henry Cavill, also Justin Theroux
    Also are mature hairlines real?

      Whatever is maintained is due to Finastreide. You need to know Finasteride only blocks 3/4th of DHT, not full DHT.

    Hey Doc, I've had a receding hairline since I was 15 and it stopped around 17 at a (grade 2-3) I started finasteride at 19 stopped at 21 and started again at 24 but with no progression over the 3 years I was off of it (I'm 25 now). Over the past 6 years I've probably been on the medication for a total of 2.5 years. The men in my family are nw3s with thick hair so does that mean my temples are sensitive to the DHT but the rest of my scalp is not (or less susceptible to the effects of DHT) and should I continue Finasteride? I tolerate the drug well and its fairly cheap so I think I should remain on it due to the unpredictable nature of MPB. I've still got a good head of hair (as you can see on my avatar) but my temples are quite deep like my dads and my uncles

      You should be on low doese Finasteride since your age is ideal and you have a robust mind and body. Baldness when running in the family is always a grade or 2 higher than in the previous generation. History of baldness in family though helpful in assessment and planning trreatment is not accurate regarding when your gene will switch off.

      @@hairtransplantinindia That makes sense as my Father’s hairline is similar to mine but I’ve obviously started the MPB process earlier and it’s the same with my other cousins on my paternal side as well. I have more androgenetic hair than my father had so that may be one of the reasons my recession started earlier and slowed down since the temple hair-loss is a trait on my fathers side as opposed to severe baldness. However due to the unpredictable nature of MPB I’d rather preserve the hair I have which (apart from the recession) is a good head of hair. There is definitely a gene for hair loss (albeit mild) in my family but we all have different hormonal profiles which may express my genetics in different ways. For example females may have DHT sensitive hairs but the baldness is not expressed due to their low androgens but if they begin treatments to raise testosterone levels they may very well activate the gene as seen in gender-reassignment, menopause or polycystic ovary syndrome

      Yes. Once you take a break of a fortnight and there is no excessive hairfall, continue for another 15 days without the drug. If there is no hair fall still that is alarming, it means your baldness is stable.

      @@vishweshpandey1112 We apply Finasteride topical in the hope that it will reach the root directly and not through the blood stream like the oral formulation does. But it is not so. Finasteride is absorbed in small percentage by the skin and also reaches the root through the blood stream and reaches other tissues too since it becomes systemic.

      Many private dispensers are giving it. Also comes in combination with Minoxidil. I do not recommend topical Dutasteride so I will not know.

      @@zyzzcommunity8550 In combination with Minoxidil it is freely available. If you wish it alone, you can get it from a local compounder. However since I am not convinced by the efficacy of topical DHT blockers, I do not prescribe them and hence do not know the source of supply.
