Most portrayals of the Virgin Mary at the time of the Nativity, when she'd have been about twelve. All versions of THE HUNCHBACK OF NOTRE DAME; Esmarelda was a teen and Quasimodo twenty.
A near miss entry. Warren Beatty wanted to play the role of Dirk Diggler in Boogie Nights. He was 60yrs old at the time, and Dirk was a high school dropout at the beginning of the film. It of course didnt go to him so it cant make this list, but if it did itd have been the widest gap.

Agreed, Travolta was 23 during filming, though some of his costars, namely Michael Tucci and Stockard Channing, were already in their 30s by then.
