  • This video shows Dr. Andrew Elder and Dr. Junaid Zaman who both trained under the UK's system discuss and later demonstrate clinical assessment of the physical exam.
    On September 28th & 29th, 2015, the Stanford Medicine 25 team hosted the first annual bedside exam symposium for clinicians and educators. The purpose was to share how we do bedside teaching and allow them to improve upon their bedside exam skills.

    Lived in Scotland with my family for a couple of years while I pursued my PhD... Best care I've ever received, best care my family has ever received. My wife who had her master's in occupational therapy also worked for the NHS after a year living there, and found their training to be reflective of the best practices she learned while getting her master's degree. We pray everyday for the NHS model here in the United States, as opposed to a medical system here in the USA which would say things to her like "well if you don't fix the problem with the patient in 3 days, too bad, because insurance won't pay for more"... The wealthiest nation in the world's citizens deserve much much better given The wealth those everyday citizens have generated, not to mention the moral implications.

    Fantastic examination skills, shows the depth of IM/general/musculoskeletal expertise in the Western European methodology. A lot of what they assess during calibration is almost a perfect hybridization of DO and MD emphasis. Regardless, lots of good stuff for anyone going into a clinical assessment at really any level.

    In my career I have cleared both UK exams(MRCP UK +Neurology SCE- all First attempt) and finished all parts of USMLE(Step1+Step2+Step3- First attempt). I will say there is a vast difference in study approach and both are unique in so many ways. Especially Step3 USMLE is heavy biostatistics based exam on day 1 and PACES is a pure clinical skill exam on real patients with active illness. MRCP-UK according to me the best Internal Medicine exam in the world which is adequately standardized to the level of PG IM Training however USMLE is overall the toughest exam in the world due to the sheer intensity of exam stipulation such as number of questions in each single hour section of the exam as well as the length of clinical vignettes hence time plays a key role in solving a USMLE question. USMLE feels quite tough to pass or score high per se especially at later stages of career when one is already at a level of a resident or registrar as it is difficult to conceptualise other specialties like OBGY/Gynae or Peds whereas MRCP-UK can still be cleared if an individual has a very good knowledge of internal medicine specialties. I hope some basic facts could help vloggers and bloggers reading my comment post and best of luck to anyone endeavouring the hard journey of medicine.

    PACES in details looks unpractical in daily physician's work either in medical ward or outpatient clinic to be carried when u have only 10 minutes for each patient (taking some history ,doing brief physical examination , checking the lab results and answering the patient's questions ) , but yes suitable for training's issue

    cerebellum test one could expect can fail in laying position due to gravity fighting motion, surprising to me that he did that way is much more telling in standing position which is more natural where one using hands in or as pointing tool..

    I wonder whether the U.S. or the British system produces better physicians. The French system seems especially rigorous and some of their surgeons seem very impressive. The German system seems to produce very good researchers as well as subspecialty practioners. Would anyone care to comment?

    the most important & missing from the whole thing & all doctors performed tests, including hospitals is proper blood pressure measures of both hands & related to influence of present outside weather pressure on patients (its beneficial to know behavior history of the patients blood pressure in that relation) its good to know if patient has patterns of low or high blood pressure. As we know that outside weather pressure will have colossal influence on patients cerebellum functions ..hehehe

    British doctors (especially from Edinburgh! as Dr Elder) are way better in clinical exam than US physicians.

      Physical examination is a relic of the past. Only relevant in the context of third world countries which lack adequate access to medical technology. It will become even more anachronistic as technology continues to develop at a rapid pace. Give me a US doctor's research/empirical skills over a British doctor's "clinical acumen" anytime.

      @@srw6772 I'd prefer to have a quick physical exam to narrow down possibilities that could then be tested than just have random tests thrown at me, US doctors tend to do that so it costs more I've witnessed

      We learn physical examination religiously and once we become doctors we won't use like 80% of it.
      Abdominal examination for example once you go to a doctor with abdomen problem the 1st thing he'll do is ultrasound and we don't learn how to read ultrasound 🤦🏾‍♀️
