    I have a resident who had fallen before he was admitted to our facility. He dragged himself on the ground for two days because there wasn't anyone around. He sustained abdominal injuries. After he was admitted to our facility, the wound to his abdomen began to develop slough. The nurses are documenting unable to determine.? Describing the wound. Does this sound like accurate documentation?
