I got jaw surgery with Dr Alfi in 2023. I’d highly recommend him to anyone that needs it. It is so highly customized to the patient and was life changing for me in many ways.
Now I am convinced to go with Dr.Alfi for single jaw surgery. I was incredibly hesitant because of how fast his procedures were. Now, it all makes sense. Thank you. Blessings.
Excellent interview. I’ll definitely be sharing it with family and friends. I have DOME set for October 2024 and have consulted with several jaw surgeons who have said, “I dont recommend MMA, it’s just too invasive for mild to moderate sleep apnea.” yet I’ve truly felt that it is the right route for me after the DOME. When you know, you know but it just seemed like I was having to plead my case to surgeons all while they are looking directly at my class 2 overbite and recessed mandible. It literally feels like I’m being gaslit by surgeons who basically don’t have enough confidence in their technique to be willing to work on a healthy 53 year old female that wants to continue to live a long and healthy life. What they all agree on is that my palate is extremely narrow and as Dr Alfi said on this interview, sometimes it’s just best to do palate expansion first for very narrow palates. After the DOME, I’ll definitely be making the trip to Texas to consult with Dr Alfi. Again, this was an extremely informative interview and well worth the 2 hours in length. There is just so much to this topic to discuss that even 2 1/2 hours is essentially still just the tip of the iceberg.

It seems that David Alfis perspective on what’s acceptable esthetically is what looks more normal and not what looks amazing. A lot of people are trying to also get the maximum esthetic benefit and not just look normal as David Alfi says.
I started watching your channel several months ago this year and Learned about MARPE and tongue tie I got my tongue tie released in April I’m about 3 weeks into my Marpe. The third step is Jaw surgery and I’m most likely going to go with Alfi I’m eventually going to get double jaw surgery
7 years ago, I got braces to fix an overbite and a gap I had in-between my main upper incisors. The orthodontist told me and my mother that the only way to go, is by extracting two of my upper premolars. Trusting him was the biggest mistake I've ever made, and impacts me to this day. Since I got the "treatment", I've been having all kinds of headaches, breathing issues, fatigue and worse things, daily. It turned me from an athletic teen full of life, to a very, very anti-social person. I still engage any way I can in things, like I do with the community here, but my health issues have sucked almost all joy off my life, and one of the worst parts is that even after taking the time and making the effort to understand where everything originated from, when I try get help from doctors, they dismiss me. One even suggested I get the opposite of what I requested, which was a lower jaw surgery, as that chin's been pushed and locked backwards from its comfortable position. What a fucking clown. I wish I could get the help I need from someone like Dr. Alfi, but my broke ass can barely afford to live, so I've gotten comfortable with my awful position. The moral of my story, is to not always trust a doctor. Even if they have a degree. Sometimes, not only does that mean shit to their actual competence on their science, but it's actually a harmful element, as it can lure and misguide clueless people, into a horrible, horrible path. Beware.
This might sounds crazy, but if you aren't completely screwed with fitness, you can get a job fighting wildfires for the forest service. You can get by on a handcrew and make 30k of disposable income living out of govt provided housing, which is extremely affordable. You could also get blue cross blue shield ppo insurance which Dr. Alfi accepts. In 2 years you could easily afford to have the surgery done and change your life. Might sound extreme, but I am doing exactly this. I hope you find relief from this ailment.
Amazing interview. Alfi is a badass and super thoughtful as well. One huge question for me is: why hasn’t he done MMA himself? If it’s so amazing it seems like a no brainer and he often talks up his doctor partners in his practice. So it’s a huge puzzle for me that he hasn’t done the biohack he’s pitching. Still I might do MMA with him at some point, but it’s a little weird he hasn’t eaten his own cooking so to speak.
I can tell you why. I asked his brother who works on him team this right after my consultation with Alfi. He simply doesn't trust any other surgeon can do the job he does, and he can't do the operation on himself. So there you go :)
@@waveypizza I figured that might be the case. There’s also the aspect where I’m sure he’d lose a lot of income for himself and others with a recovery, which is totally valid. His opportunity cost must be high. But that said, I’ve heard him hype his surgery partners before publicly, and that doesn’t add up if he trusts no one else to do the surgery. Idk. I’m not criticizing him. It’s more I can’t understand why he hasn’t had one of them do his surgery if the benefits are so great. He’s not an ugly man but he doesn’t exactly have Margot Robbie jaws, no what I mean?
@@Thinkagain1234 Yeah, perhaps it's not 100% fair I said that given I didn't ask him the question directly. And when I said "doesn't trust" I think of it as anyone who is extremely talented and confident in their craft who has to relinquish control to someone else who they don't believe can do exactly what they do. I think of as a fine artist who creates most of a masterpiece painting, and then has to turn it over to another artist to complete. It's going to be tough to do that. Even if on paper the operation is the same. I do see your point as valid, but Dr. Alfi comes across as someone so driven and busy, does his own airway issue warrant stopping everything right now? I'm not sure how serious it is for him. I'm more inclined to scrutinize the results (visual and anecdotal) and their demeanor when coming to a conclusion about a surgeon

I guess I’d like to see him say “well I have a 0 AHI and RDI and don’t have time to do a surgery for the marginal benefit I’d personally get” or whatever the answer is. In any case if I do MMA I’d be most likely to go with him. I’ve also consulted with him. What was your consult like? Mine seemed a bit rushed and he didn’t seem to focused on my case but then I may have caught him on an off day. And it’s not like he was terrible, just a little distracted
@@Thinkagain1234 Yeah, that's a valid point, to hear him address that point. I had the same experience, I was really tired from flying in early in the morning and waited around 4-5 hours before seeing him. I could've timed my appt better by having it earlier, but I imagined a bit more one on one time. He was supposed to come back after initial chat and answer my questions but he was onto the next patient. His brother is super cool and took the time to answer them, thus the conversation. In fairness to Dr. Alfi, he saw my case as pretty clear cut and was in town for the purpose of consultations with a busy schedule. But still, as a patient, it doesn't feel good to invest the $ and time, and feel like you don't get the time you expected, or left behind.
Hats off to Dr Alfi for being so open to newer methodologies that challenge the status quo. It sucks that some Dr's have their head up their a$$ and rather stick to old ways than admit there's a newer a better way. Even though this would greatly benefit their patient's outcome smh...
Hey Ron could you ever do a video with joint focused OMFS like Mohaved/Wolford? Seems it may help people that would think to go down the DJS route that have really diseased joints, but seems it is a much lower percentage of those who get jaw surgeries for joint issues.

After considering two years of MSE treatment, I’m getting another opinion on jaw surgery. My mid face isn’t that poorly developed but my lower face really is despite having cosmetic work. My airways are super narrow so I don’t see the point in doing both when the MSE might actually just make my face worse leading me to need jaw surgery anyways

So anyway my question is regarding the Shwartz and Saggital Appliances and whether the expansion from those is mere tooth or bone. From my quick reading it looks like both but I am skeptical. I also dont trust Alf appliances. I dont understand how those change anything with that small of a wire. Also when using expanders would you encourage some sort of bone growth supplement to help the process? Maybe even achieve more this way?

This is so funny how much people know and talk about their own surgeries and get really into it because the people have consulted with make it just seem like we’re gonna do this and that and OK we’ll see you when it’s time and I’ve talked to the top three surgeons in California Number two. I’ve talked to some surgeons and or instructors from other countries but the scary thing is I really believe in other countries they just have a whole different concept of the surgery for example, you can’t even talk to them about VSP and computer simulations and custom plates. Unfortunate because they’re the leading experts in their field and they think you’re crazy for doing this, but they have no idea what’s insulting is when you know a little bit about what you’re undergoing, talk to you you are joking. You sound like you’re a doctor they don’t seem to understand no matter what someone is having this invasive procedure on their face involving their mouth nose and airways. Of course anyone with half a brain cell would research the living daylights out of them.
What about having metal plates stuck in your mouth? Would the metal not leach and cause gut issues/auto immune issues? Cant be healthy to have metal in you?

And two - in regard to condyle position. Had ortho when I was a kid, changed my bite, brought my mandible forward (condyles in a more centric position w/in the mandibular fossa). 2016 had a poorly fitted night guard made that completely fkd my bite - drove my jaw back to a posterior position. Since, my clenching, headaches etc. have all gotten MUCH worse. Can this be repositioned via mma? Basically looking to create a slight overbite to allow the mandible to come forward again.
Can’t trust this guy. He has those “I caused great harm to my patients and attempt to self deny it” eyes. Very specific eyes that only sht doctors develop. A mix of guilt and extreme denial.
28:28 did he really say "I'll probably operate on both of my brothers" ? In the entire medical community, across all specialties, it is heavily frowned upon for any surgeon to do any operation on friends or family. Yes it's technically allowed but I do not know a single surgeon who believes it's a good idea to do that. Great interview overall, but this one statement really stands out as fairly uncomfortable to hear from someone like Alfi with such a strong trustworthy reputation.

I didn't hear anything about if it’s necessary to remove wisdom teeth before Jaw surgery ? Do you know if Dr Alfi require that
I asked about wisdom tooth removal on his Instagram account and was told it isn't necessary. I'm sure it's a case by case basis, of course.

In the entire medical community, across all specialties, it is heavily frowned upon for any surgeon to do any operation on friends or family. Yes it's technically allowed but I do not know a single surgeon who believes it's a good idea to do that. Great interview overall, but this one statement really stands out as fairly uncomfortable to hear from someone like Alfi with such a strong trustworthy reputation.
