Doc, is it rheumatoid arthritis what I am suffering of? One doctor told me this is psoriatic arthritis. How do doctors know which is which? I am Dr. Hatem Eleishi, Professor of Rheumatology at Cairo University in Egypt. Stay with us while we answer this question in this video. If you have not subscribed to my channel, please do now. You will watch a lot of videos that will help you with different types arthritis. There are several symptoms and signs that can help us sort out rheumatoid arthritis from psoriatic arthritis. You don’t have to have all those symptoms and signs together. You need to have some of them. We weigh the strength of each of those symptoms and signs in the process of sorting out rheumatoid arthritis from psoriatic arthritis. Number 1: the pattern of joint affection. Rheumatoid arthritis tends to affect the joints of both sides in a symmetrical fashion unlike psoriatic arthritis that tends to be asymmetrical. This is not a rigid rule though and exceptions can exist of course and it’s up to the evaluation and assessment of the doctor to what extent he/she can rely on that sign. Number 2: Psoriatic arthritis, in one of its patterns of joint affection, loves to inflame the distal row of the joints of the fingers and rheumatoid arthritis does not show much interest in that row of joints. If this is the pattern of joint affection in a patient with psoriatic arthritis, it really helps a lot in the differentiation process. Number 3: In fifty percent of psoriatic arthritis patients there is not much stiffness of the joints in the morning unlike rheumatoid arthritis where morning stiffness is a more recognized symptom. Number 4: Sometimes, patients with psoriatic arthritis will get what we call sausage digit. This is inflammation of all the tissues of a finger or a toe. The joints and the tendons and even the soft tissues of that finger or that toe in that case are swollen and look like a piece of sausage. This sign, if present, is very much diagnostic or suggestive of psoriatic arthritis. Number 5: Having psoriasis of the skin or nails or scalp. If you have psoriasis, skin psoriasis for example, it is likely that your arthritis is psoriatic arthritis. But, again, this is not a rule. We cannot confidently say that any patient who has psoriasis of the skin and complains of symptoms in their joints that they have to have psoriatic arthritis. They need to be assessed for that and as I mentioned the doctor will do that assessment and decide. Number 6: Imaging: we can do some imaging studies.

Hello doc! On left thumb ip and mcp joint hurt but not on right thumb, I also got couple of skin problems I never had before. Like a huge rash on my bicep. And very small but quite a fewlittle blisters on palm. Both on right hand but joint pain is in left thumb.
Its been 13 days and left thumb alone hurts so i can rule out RA right? Since its symmetrical. Please help me I'm worried to death. do you think it's RA?
You need to be seen in a rheumatology clinic to be examined and this can be sorted out then
I think I have psoriasis when I look at my skin and nails, but the arthritis in my knee, ankles and feet very much looks like regular arthritis. My upper limbs are fine. It's painful to sleep, and to get up in the morning. Pain and discomfort is always there, but slightly lessened during day. I don't take and pain meds, as I want to have a clear head. I accepted my fate...there is no cure for arthritis. Pain meds will destroy my organs and cause me to bleed internally.
I think you have misconceptions about arthritis and its medicines.. I hope your arthritis is not that severe because if it is severe and you don't use medicines it might progress to complications
Greetings from Alaska Doctor, I have had moderate psoriasis since age 2 and now 48. Have been warned for decades to watch for signs of PSA and am having the first bout of it in the toes on my right foot in the joint right under the nail. I’m not shocked I have it; I’m shocked at how painful it it. So, I changed up my diet and the inflammation has subsided by about half. I can wear socks again. I’ve known people over the years w/ RA and it seems like their symptoms haven’t abated much, no matter what they do. Medication is required. I am thinking PSA is a little different - it is more episodic and triggered by things that cause stress …. the same things that cause my psoriasis to flare-up. Is there some truth to my observations? Thank you.

PsA is not the same thing in all patients.. It can be different... it can be mild and episodic like yours and it can be aggressive and progressive in other patients. I am glad your PsA is getting under control with diet adjustment.. Make sure your rheumatologist also confirms it is mid and that there is ongoing inflammation going on with little symptoms as this also is sometimes another possibility in PsA

Hi Doc! Hope all is well with you. I started to have a symptoms just last month. First i had eczema. Then i began to have a pustular psoriasis. Then followed by arthritis. It’s so painful. And i’ve been bed ridden because my knee is so stiff While both of my hands/wrist and also my left hand is very painful 🥺🥺🥺

I hope you're doing well. I have it kind of backwards. First thumb pain. Now I'm seeing some skin problems it's been 13 days. I'm so confused and worried my small town doesn't have any rheumatology specialist.

My mother had rheumatoid arthritis
Recently I had a lot of pain in my neck and back and all my body specially at night that I can't sleep
I went to the hospital and they asked me to do blood test and x rays in order to determine if it RD arthritis or psoriatic arthritis
My appointment will be next month but I just received the results

Hello Halal.. those blood tests do not sort out.. what will sort out can be the pattern of the arthritis as described in the video, affection of the sacroiliac joints (the very lower part of the back) as confirmed by X-rays or better by are MRI

Hello Chistopher.. you don't have to have sausage fingers or toes to be classified as psoriatic arthritis.. these are only helpful sigs if they are present..
Your doctor will sort out your diagnosis whether it is psoriatic arthritis or rheumatoid arthritis and he should tell on what basis he made the discrimination of course
