I’ve been struggling with knee pain since I was ten, I’m 28 now and it seems to get worse and worse. I was an avid skier and cheerleader into my late teens. I got diagnosed with patellofemoral syndrome about three years ago. I thought finally my pain will go away! But the physical therapy hasn’t helped and it’s to a point where they get swollen and I have to ice them almost every night. The X-ray I had three years ago showed my patellas were both off center, leaning towards the exterior of my legs. Is there anything else I can do besides manage pain? I can no longer ski, bike, lunge, or even manage a flight of stairs without being in considerable pain
you should try walking backwards, it sounds insignificant but it’s one of the only things that’s helped me with this condition, I hope this helps you get back to what you love
when I had it many years ago I found this helpful: strengthen VMO (the lower part of your inner quad), stretch out hamstrings (but only when the muscle is already warmed up), also stretch out butt muscles and calves, and finally, when walking, try to remain very upright with like a 'book on your head' posture, keeping abdominal muscles and butt muscles activated while walking. I found that really helped me a lot. I haven't had that problem since
Yup I have this, had all the tests and they say do the exercises, which I have and it still isn't sorted. Walking for too long, bending going to the loo as in going up and down the stairs just kills me.

I've been through two orthopedic doctors and PT's, and they haven't even come within 1/10th of the level of detail and thoroughness as Paul. The orthos looked at my knee for about a minute, mainly just ruling out meniscus and other major tears (not really trying to actually diagnose me), and the PT's just throw template exercises at me without focusing on a plan tailored for my specific PFPS diagnosis (that hasn't even been established). They just do what most PT's do, stabilization, hit the VMO, hit the hips, not providing any at home regiments... Pain isn't going away. If only Paul was near me.
It’s hard to find a good pt. I just spent loads of money, post op TKA on an average pt. Got most useful information for my home therapy regimen from UA-cam!!
i have lateral patelelar side discomfort in left knee. please make video of exercise or suggest any alreadymade video. is there any other treatement other than exercise pain killer. please guide me
Hi doctor kindly reply, diagnosed with pfs in left knee for past three years, xray shows patella is little above to the knee, doctors have given nsaid and vitamin d3 tablets, nothing works so much. Whenever the sleep is less the pain in knees is more and the pain is more while bending. Exercises doesn't help much. Now another doc has advised me to take glucosamine tablet. Can you recommend on how to proceed further....... Thanks.
There are so many causes of pain in that area that it wouldn't be fair to give advice on something I haven't evaluated. Have you been to an orthopedic surgeon to look at it?
I have this, yes I was diagnosed. I have stretched and exercised many of times and nothing has helped. I also went to physical therapy and that didn’t help either. They told me this wouldn’t be a long term issue but it’s been 2 years sadly and it’s extremely painful In the winter!
I've been diagnosed with PFPS as well. It truly sucks. Such a drawn out thing. I've been seeing my physical therapist for 6 months now and we've gotten basically nowhere. We're going to start from scratch next week and focus on the things which seem to have helped somewhat. I've been x-rayed for it and they found no signs of arthritis.
@@michelegiannone7403 I'm still struggling, although I have made some progress. I still have pain and really want to get back into running- how long until my symptoms start to go away(,:
I’ve been dealing with this for a long time. Be prepared to give up high impact (running, jumping) for at least 6 weeks. I’m at the point where I can’t even do quad exercises at all because the pain is that bad. The key lies in most likely your hips, ankles can play a part too. There are 4 things I do daily that are helping immensely: 1: slow backwards walking on an incline treadmill (10 mins) 2: lying leg abductions for glute medius 3: banded clamshells for glute medius/tfl 4: whatever quad loading you can handle, for me it’s straight leg isometric holds and straight leg raises
Wow! I had amy first (and only?) appointment at Twin Cities Orthopedics (MN) a couple days ago. In less time than it took to watch this video, I was diagnosed with patellofemoral arthritis and out the door. Too bad Maine is so far away. I'd be there for a second opinion tomorrow!
I also have the same issue after I pushed my car up the hill alone, I feel a sharp pain at the top and inner part of my right knee whenever I want to run or if I put too much stress on the leg, I will feel much pain the next day but if I did not put much stress on the knee during exercise, I don't feel pain the next day. The crazy part about it is, if I did not exercise for a month, the pain gets worse from the top, both sides and down of my knee when I massage it..
I think this is what I have. My knee clicks when I walk and when I try to massage it I feel like something moves and it hurts a lot. When I sit for too long it hurts I can't wear my heels for too long, I limp and I can't get on my knees the pain is bad. I also can't put my leg back to stretch 😫. The examination you did here is far more than what my physician did. 😒 All he gave me were anti inflammatory and pain killers.
I wonder if you could advise - I have been having some knee joint pain and I think it might be Patellofemoral pain syndrome - squatting doesnt hurt - nor does bending the knee and most of the excerises you did dont hurt. The only time I get a sharp pain in my knee is when you did the hyper extension when my leg is dead straight I get pain in the front of my kneecap. I have poked the cap, moved it etc and bent it and no pain in other ways - it does also happen a bit on the excerise you did with him standing on one leg and putting pressure on it :( cant go to the doctors at the moment due to lockdown lol!!!

I just saw this video.. I have a question that u asked the shane to tighthen the muscle by holding patella at around 3.30 mins and he didn't had pain., but in my case, i had v severe pain due to that test.. Whats that called?..
Firstly, thank you for this amazing and clinical eval sir, just had a few questions being a physical therapist assistant student. So there will be lateral patellar tracking most prominently with reduced medial patellar mobility?
Yes typically but not always. The first of the quad muscles to shut down is the VMO. Therefore the quad pulls the patella more laterally and the most common area of discomfort is the supra-lateral patella where it contacts the lateral femoral condyle.

Also if I’m sitting down for a while and I stand up I here a large crack from it and hurts a little when that happens I’m curious as to what it is.

There are many causes of this type of pain so giving an exercise program would not be wise. I would recommend meeting with a PT to help develop a program customized to your needs.Best of luck!
I was diagnosed with this in my right knee in the military 19 years ago... Now, I'm presently in a wheelchair because my legs can't bear weight, are both giving out on me and are spasming in pain. Does PFPS actually worsen over years and become more than pain, like this, or is my diagnosis more likely just another product of a broken VA system?
Hi Keegan, Absolutely! Reflex inhibition plays a major role here. The VMO shuts down first along with lateral retinacular tightness leading to too much lateral movement. Check out this video. Classic. ua-cam.com/video/FF4N5EYNtvc/v-deo.html Best, Paul

It’s been 2 years bro did you get through it?
What exercise should i do with this??
I've been through two orthopedic doctors and PT's, and they haven't even come within 1/10th of the level of detail and thoroughness as Paul. The orthos looked at my knee for about a minute, mainly just ruling out meniscus and other major tears (not really trying to actually diagnose me), and the PT's just throw template exercises at me without focusing on a plan tailored for my specific PFPS diagnosis (that hasn't even been established). They just do what most PT's do, stabilization, hit the VMO, hit the hips, not providing any at home regiments... Pain isn't going away. If only Paul was near me.
Wish I could help. Hope you find someone to take it seriously. Best of luck!
It’s hard to find a good pt. I just spent loads of money, post op TKA on an average pt. Got most useful information for my home therapy regimen from UA-cam!!

I consistently work out my quad muscles with no relief, I just started foam rolling the IT and vastus lateralis. My ortho told me to work on VMO strength so I’ve also been trying to activate that with my warmups. I still have pain and really want to get back into running- how long until my symptoms start to go away(,:
Same question here ! One month with no improvement so far !
I’ve been dealing with this for a long time. Be prepared to give up high impact (running, jumping) for at least 6 weeks. I’m at the point where I can’t even do quad exercises at all because the pain is that bad.
The key lies in most likely your hips, ankles can play a part too. There are 4 things I do daily that are helping immensely:
1: slow backwards walking on an incline treadmill (10 mins)
2: lying leg abductions for glute medius
3: banded clamshells for glute medius/tfl
4: whatever quad loading you can handle, for me it’s straight leg isometric holds and straight leg raises
Yeah man I got the same thing from running as well it’s really depressing not being able to run without it flaring up
How are you feeling now
I feel this a lot in my 10-month post-operated patella tendon rupture. Would you say this is common after that surgery?
I also have the same issue after I pushed my car up the hill alone, I feel a sharp pain at the top and inner part of my right knee whenever I want to run or if I put too much stress on the leg, I will feel much pain the next day but if I did not put much stress on the knee during exercise, I don't feel pain the next day. The crazy part about it is, if I did not exercise for a month, the pain gets worse from the top, both sides and down of my knee when I massage it..
For warmth aren't we supposed to use dorsum of hand for better temperature sensation??
Did you ever found out what's wrong? I have that same issue on one kneee

Does it ever fully heal after you do the stretches and strengthening?

I was diagnosed with this in my right knee in the military 19 years ago... Now, I'm presently in a wheelchair because my legs can't bear weight, are both giving out on me and are spasming in pain.
Does PFPS actually worsen over years and become more than pain, like this, or is my diagnosis more likely just another product of a broken VA system?
Hey doc....what if this pain is in the medial side of kneecap....what can be the possble reasons?
Could be a plica. or patello femoral pain.
Do you find that focusing on quad strengthening achieves quicker improvements than gluteus medius strengthening?
Hi Keegan, Absolutely! Reflex inhibition plays a major role here. The VMO shuts down first along with lateral retinacular tightness leading to too much lateral movement. Check out this video. Classic. ua-cam.com/video/FF4N5EYNtvc/v-deo.html
