If she answers in the affirmative then you serve it before starting the procedure proper if she says no then there's no need to serve the bed pan. You have to ask the patient if she uses soap on her face and act per her response. In this, the patient said she doesn't want soap on her face that's why the nurse cleaned the face with only a wet towel. So washing the face is done per the client's preference.

I don't know how u were taught but mouth care is not part of bed bath and the rest u are saying is not in line. Find or ask ur teacher for the component task on bed bath to confirm. (Note that my videos are according to the NMC Ghana component task, ur country may have different guidelines)

The bed pan is placed purposely for washing the groin including the vagina and the anus, in order not to wet the bed. Every nursing student who has done bed bath knows this. So it was done OK?

Wash perineum and groin with sponge and soap. Rinse vulva with only water and do vulva toileting with cotton and diluted savlon.
