An upper entrance only serves to allow heat to escape and condensation to form as the cold air enters. Condensation forms on cold spots, so if you can eliminate the cold spots then you eliminate the condensation. He has some great videos on the benefits of heavy insulation and no upper entrance heat escapes.
Bottom entrance is 2-3 inches wide and maybe 1/2 high.. I use an open screen bottom board. This year I am trialing fully open screen vs 1" inch open. Previous have always been fully open into a protected space.
not too bad... the reason I add a honey super without excluder is to allow nest to expand up, bring nurse bees up into the honey box. I then add the excluder to allow brood to hatch out and hopefully back fill it with honey.

Great info ! Do you winter all the hives in singles I am thinking about singles this year in 1/2 my hives I am only a hobby keeper
Mostly... I will occasionally winter doubles, but my preference is in a single box. Makes for easier spring management.
