In my opinion it happend because of high spindle speed. And as for me this cartridge not for this lathe machine it's so big. And very big distance between cartridge and toolholder.
Had a lathe accident last week, it rolled up the sand paper and my gloves on the piece I was polishing, luckily it ripped my gloves out and i was able to take my hand out only with minor scratches and a black nail on my thumb.
No gloves. Use file for debur and then polish with sand paper at low rpm. Not even the latex gloves. I often use and work with what I have, until one day and then I will ask myself, was it worth it? The biggest close call was at work as a college student. I could have broke both of my legs, maybe even sewer them. The forklift fork caught doors of the container. It had a huge shaft for closing them. The shaft tore my working pants. Didn't catch my legs. I am amazed by pure luck that was with me on that day. I am often into a black hole of bad thoughts, but I have more good things that are pushing me.
@@MarioAPNI've been using latex gloves, they are very safe, they sometimes get caught but they break very easily so is very safe to me. Unfortunaly i can't use files or low rpm because i need to make a mirror finish.
Not if the nuts were drawn up tight (they weren't); The only thing the studs and nuts are supposed to do on these little lathes is to hold the chuck against the registration diameter on the spindle face. Had there been audio, we would have heard something VERY amiss from the first cut attempt.

Bolt on chuck with loose bolts. From the get-go it was obvious something was going to let go.
In my opinion it happend because of high spindle speed. And as for me this cartridge not for this lathe machine it's so big. And very big distance between cartridge and toolholder.

it's very risk to use gloves with rotating moving parts, glad that you are safe

There is a rule to never wear gloves near rotating machinery!
Chuck way too big and heavy for the small studs used to hold it on the minilathe.

You were spinning it too fast, no tail stock support, and the tool was either too high or dull. You caused this accident.
