At the end of the day, I'm still not 100% sure, the one thing that convinced me that this dialogue is not accesible is that one where in the previous patch (patch 5) is still bugged out, but after patch 6 they fixed it. If you actually managed to get this without using any mods, please let us know in the comment.
By the way, the reason why I use clothing mods is to differentiate between 2 different playthroughs.

Just finished mc Wyll X Karlach run . Ended up together in Avernus, but it was worth it! Also, if you ever end that route, check Wyll clothes during epilogue meet. I had a LOL!

Is it possible to get some revivify scrolls before this? Is it possible to be Origin Wyll on multiplayer (as a host possibly)? Is it possible for multiplayer players other than the host to recruit party members? If so, have you tried:
multiplayer playthrough with 1 player being origin Wyll
kill Wyll and recruit Karlach with player 2
revive Wyll and kill player 2 so that no Tav (or anyone) would interrupt
talk to Karlach at camp as Origin Wyll
(I'm grasping at straws here, I don't own the game but this sounds possible if all of the above questions have the answer of "yes")
