Good one, doc. There are a few legal battles going on out there on two twists to the story. First are the battles over the funding of "premium paying charities," and then the one where employer plans (self-insured) move to a reference base provider payment model where they only pay Medicare or some low (125-150%) multiple of Medicare for dialysis services. I'll bet BH throws its weight around doing all it can to keep those commercial rates for DaVita. Ultimately, one hopes, common sense over what a "fair" rate is (is it what Medicare pays?) will bring some sanity to this dichotomy of pricing.
for every dollar DaVita or Fresenius donates to the American Kidney Fund, they get roughly $3.50 in return from private insurers. the two dialysis giants, which together earned about $2.2 billion in net income in 2019, reportedly donated $247 million to the nonprofit organization in 2018 - roughly 80 percent of the fund’s annual budget that year.

I am seeing many self-insured employer groups paying significantly more than $149k. If they do pay only $149k, it's because their spec deductible has been reached and the reinsurance is paying the difference. Many that I work with were paying from $300k to over a $1 million per member. I had a client paying over $2 million (in 5 months) for a member before they started working with our firm.
Is there an anti-competitive dynamic that prevents new entrants to the market? It seems like employers would be jumping to contract with lower cost dialysis centers. Or does it just cost a lot of money to run a dialysis clinic?

Hi Eric, I'm somewhat new to the Strategy side of healthcare so bare with me.... Are the local dialysis centers corporate owned typically, or independent franchises? I've tried to go up the chain of management to reach decision makers to learn more about whether or not they are involved in "managing lives" or Value Based Healthcare as I understand at one time CMS was experimenting with ACO style funding due to the extreme expense these patients present. As a leader in a nationwide vision care provider group with over 3000 independent locations, I thought a relationship with these entities could improve their HEDIS/Star positions with improved Diabetic Retina Exam scores and related eyecare needs. Any thoughts on how I might reach persons in those organizations interested in learning more? Is there a way to engage you in Direct email or Private messaging?

Look to the connection between dialysis companies and Nephrologists. Need the nephrologist to write the dialysis orders.
