The south park creators didn't take into account that people would be too shocked that they didn't hear that this doesn't affect gameplay in the slightest, haha.
I think I heard it affects shop prices and stuff. Generally everything outside of combat in the game.

It affects loot drops in the overworld and shop prices, so it is actually more difficult, but the enemy HP and damage doesn't change.

They did it entirely as a joke, which of course only a game based on a show of satire comedy could ever hope to do without Twitter blowing up about it.
