What should be entered as an entry date in the e-visa application form, departure date from India or the arrival date in Vietnam just in case the dates are different?
it must be arrival date always but some airlines staff lack knowlege and rather try to find data of departure. So choose a good airlines. I had specified my data of arrival the day I arrived and I had no issues

You still can get Vietnam e-visa with your single name on your passport and it is still accepted. You also can get Vietnam visa letter to collect visa sticker on arrival.

you can apply your visa 15-20 days days before your schedule departure.
I was denied boarding for the same reason in last December. Booked it again in 3 days after getting approval letter via an agent.
