You need to declare the port you are entering but also the port you are leaving from. If you decide to leave from a different port than you declare on the application does that cause a problem?

Hello! You'll need a multiple entry visa. You only need to list the first entry point where you plan to enter the country. This is because the visa allows you to enter and exit Vietnam multiple times, and the subsequent entry points (for your second, third, etc., entry) are not required to be specified at the time of application.

Not exactly true about Canadians requiring a tourist visa. I'm currently on Phu Quoc Island. As long as I stay on the island, no visa required.

Multiple entry is if you fly into vietnam and spend a week there but then you leave for Japan for a few days and then you fly back to Vietnam.
