This is basically a slight overexaggeration of what German hospitals really are like. You usually share your hospital room with 2-3 other patients and there's always a morning routine where the nurses and doctors go through their daily schedule. Needless to say, your sleep tends to get interrupted a lot as a result of this.
Yeah we do have free healthcare in germany and on a site note he was in the hospital because he had a crash with his motorcycle beforehand, the guy drawing this took his stay in a hospital as an inspiration for this scene, since it was essentially a true-to-life caricature, because he was in the hospital for the same reason Werner was. At least it was like that in the movie and for the purpose of the movie it was shown as such. The animated scenes are overexxagereted to 11, but the morning routine in hospitals is generally not that great. Fun fact about the movie is, that its most popular for the animated scenes, but alot of it is Live-Action.

Werner was cult cartoon in the Nineties (hence not what is politically correct these days😉) and reflects what life felt then, in the North of Germany. What Brösel, the cartoonist, got well is that compassion is often missing in German hospitals. I noticed the difference in Britain while I was living there at that time. The standards are much better in Germany-and free, but the compassion is often lacking, while in the UK the standards were low (no money in the NHS, these days it is even worse), but the nurses were and probably still are very compassionate. My husband actually learned German with Werner cartoons in the Nineties in Britain. No Internet then, I had bought them on VHS. These days we actually live in the North of Germany.
