It’s up to you on what you want to put. But including that you were previously denied a visa would not prevent you from getting a UK student visa if you meet all the requirements.

especially for US, they may likely do a background check. If the rejected stamp is on your passport, just state the reason for rejection even if you don't have the letter.

I don’t think you can make payment before you get CAS. Because once you make payment, you can no longer change anything in your application.
