The toll operator would write the number calling and the number called on a card before she made the connection with cords in outgoing trunks. The card would be put into a calculagraph which would stamp the start time. When the call ended the the lamp would go out on the outgoing trunk jack and the operator would pull the cord and stamp the end time with the calculagraph. The stamped card would be sent to billing.
I never placed a long distance call through the operator, but I know the system was around when I was a kid. We went to an open house at the local United Telephone System office, a lady from our church was an operator supervisor, and she conducted the tour through the switchboard area. I remember being fascinated and I remember those clocks for stamping the tickets. And at the end I even got a ride in a cherry picker!
Whilst that was true,in that era it was the men who had the manual jobs saying that I was an Operator in the 70's and it was the men who worked the night shifts as Operators.. ☎️ it was a great job I loved it..

I can certainly remember in the early 1970's when direct dial came to our little part of Florica - on GTE. Bell was mostly on Florida's east coast.
I can bet there is a lot of mathematics involved on the search of the proper rute
