My guess is the local police officer who climbed the ladder was just checking it out and didn't call in the Special service that a suspicious person was up there. When he went up the ladder he was face to face with a rifle and fell/got down. Then the shooter in a panic shot at Trump rushing his shots and missed multiple times. So probably a really big fail in communication between local police and Secre Service.
