i will forever remember being scared as shit of ultra hard, and when i finally decided to give it a shot after hundreds of hours of playtime (because it was one of the last pieces missing for dead god), i just got a run where i had both item rooms adjacent to the starting room...
and they were c-section and holy mantle...
can't wait to do it again without that much luck :)

To counter the no health dropping thing for red hearts you could just get placenta,charm of the vampire,leech,a full health pill,and maybe like the fork trinket cause all of those things heal isaac so you wont need to pickup red hearts to fill up your empty heart containers

Rules
Before picking up any item you must first ask tim ai to give you a question, if you do get the answer right, you get to have the item, if not you must fight a Blastocyst, however the number of Blastocyst increases as you get the questions wrong, so the first question you get wrong you'll only fight, then two, 3 and so on, all at the same time

I feel like Reworked Foes doesn't really make things harder. I mean, it makes SOME stuff harder, but OTHER stuff EASIER. If you REALLY want things to be hard, try the Eternal Edition mod, which adds eternal champions of enemies and bosses, which comes from Flash Isaac. Eternal champions are WAAAAY harder than their normal versions. I installed the mod, but then removed it after I killed an Eternal Soul Sucker (the flies that shoot brimstone in four directions) and it then shot brimstone in like TWELVE directions, killing me
