For those who want to practice the video without having to rewatch it every time:
First of all, stand with your feet together (demonstrate) and close your eyes, keep your eyes closed for a moment (hold hands on either side of patient to prevent falling with a positive Romberg)
Open your eyes, can you walk heel to toe like this? (demonstrate)
Good, walk on your tiptoes (plantarflexion), walk on your heels (dorsiflexion), that's lovely.
Hold your arms out like this (demonstrate) with your palms flat, and close your eyes, keep your hands where they are, keep those fingers straight if you can, that's lovely; let's keep still like that for a moment. And now, with your eyes still closed, touch your nose with /this/ fingertip (touch patient's index finger, can be any other finger), and with /this/ fingertip (touch finger on contralateral hand).
Good, open your eyes, play the piano (demonstrate) - good; and tap your hand (demonstrate), other hand (demonstrate), that's good.
Now turn and face me, and screw your eyes up tight; lovely, relax.
Silly grin. (demonstrate) Stick your tongue out. (demonstrate) Wiggle it, wooloolooloo (any other sound is acceptable as long as the patient or doctor isn't covered with saliva), that's lovely.
Stare at my face (one elbow flexed, one elbow partially extended to create test visual fields), point to the finger which moves. (Move fingers)
Stare at my face, point to the finger that's.. that's it.
Change hands, test contralateral visual fields. Stare at my face. (if you can bear it - make typical British joke)
Point to that fingers that move.
And stare at my finger, now keep your head still and just follow the finger. (Make sure head is indeed still, test EOM, not neck muscles).
Look at the finger, follow that finger, that's lovely. (Move your finger around in an “H” pattern to test for CV III, IV, VI)
Excellent, and now can you lay down on the couch please. (?)
OK, just gonna' tap your reflexes here. (triceps, biceps, brachioradialis, quadriceps, Achilles)
Gonna' scratch your foot, you won't like this. (Pinch plantar surface)
And again (contralateral foot), lovely, isn't it. (?)
And stare straight up at the ceiling, just going to look in the back of your eye here. (Left eye, left hand, left eye; right eye, right hand, right eye) Keep looking through my head. (vision must be focused at ceiling for proper retinal visualization)
And look over there. (test remaining retinal surface)
    - Test other eye using same technique, ask patient to look the other way after staring at the ceiling.
Excellent, you passed with flying colors.
