  • Finasteride (Propecia,) a DHT blocker, is often used to prevent hair loss, sometimes along with dutasteride. But some patients experience a syndrome of sexual dysfunction including erectile dysfunction, along with issues in mood, concentration, and a sense of malaise and fatigue. These men face a choice between hair and sex. This is because DHT is a powerful androgen that, unlike testosterone, cannot be aromatized into estrogen. In this video I help guide you through the decision of whether to use finasteride, its mechanisms of action, and alternative hair therapies.     I tried finasteride over 10 years ago for my hairloss and had the worst reaction to any medication ive ever tried. I had soft erections, inability to climax, gynecomastia and suicidal thoughts for the first time in my life. All those side effects went away after stopping finasteride and going on oral minoxidil. Havent had a single side effect from minoxidil and havent lost any hair since I started receding 10 years ago. Im not sure if minoxidil would be as effective if I was using gear though.

    I’ve been on finasteride since 2005 and have had no bad side effects and my hair grows like crazy.

    Minoxidil with derma needling works way better for me than finasteride for keeping and growing hair. You don’t have to give up and let the hair go.

      Agreed. I use this and I have had great results. But I use the minoxidil orally.

      It’s case by case minoxidil and finasteride as well as most prescription hair loss medication work the same way. Topical Minoxidil really messed me had interactions with other medications I was using as well as just episodes of high anxiety as well as negative thoughts and a thinly veiled general anxiety.

    8 years ago I took it for 5 months because I thought losing my hair was the worst thing on earth. Now I got PFS and I have 0 libido and 0 erections and 0 drive for the past 7.5 years with no real hope in sight.

    Doc- what is your perspective on using Saw Palmetto & Pumpkin seed oil and their impact on DHT through 5AR?
    Is the effect similar but much less potent than a real medical drug like Finasteride?
    Would they have effects on erections also?

    1) Testosterone gets deactivated into estrogen.
    2) DHT is not crucial nor essential as people who lack the 5ar enzyme live long and healthy lives!
    3) There is some evidence showing that finasteride increases muscle hypertrophy as it increases testosterone by like 10-20%.
    4) DHT serum levels are meaningless. The concentration in the target tissue is what matters. Also it is not needed!
    The main hypothesis is that DHT serves as backup for sexual function in times of low T.
    Also, finasteride is very specific to one isoform, the one in the skin, so it doesnt do much in your brain.

    I have been on Finasteride since 1999 age (30) when it came out as Propecia. In 2013 was diagnosed with an enlarged prostate so we deiced to update my dose to 5mp of Finasteride which I still take today. At age 54 I have ZERO sexual side effects and can have sex several times per day ( with no TRT) .

    This is first I have heard that Fin/Dut are associated with dry eye syndrome. My dry eye syndrome started about 2014, I saw multiple doctors about it, and I was convinced for a couple of years that I had Sjogrens Syndrome, and I moved to a higher humidity State to mitigate the problem, none of the doctors I saw ever suggested that Fin/Dut could have caused my dry eyes. Unrelated to my dry eye syndrome I stopped taking Fin/Dut Oct 2022. Although it is something that is difficult to quantify, my dry eye appears to have improved 50% from what it was a few years ago. I am still using topical 5% minoxidil, but I have to use a non propylene glycol version, as I have an allergy to propylene glycol.

      Try lactoferrin for you dry eyes. It made mine WAY better. Also using an humidifier to keep the humidity at around 55% even in the dry winter months helps a ton. Do not skimp on the dosage on the lactoferrin I take one before every meal. And It actually make the hairloss less for me.

    Been on TRT for about 7 years. I’ve have a lot of hair loss and thinning, but I’m not willing to take something else that could have a side affect. Also hats.

    On the subject of hair transplants I was told the transplanted hair from the sides and back of had are resistant to androgenic alopecia. The hair around the transplanted hair will still continue miniaturization at its normal rate without adding in RU58841, dutasteride or finasteride. Any thoughts on this?

      true, but would like to add that the transplanted hair ALSO can miniaturize. I might be more resistent, but sure can.

    Would a topical product like RU58841 be a better alternative to using oral or even topical finasteride? RU58841 is not a 5-alpha-reductase inhibitor. From what I understand 5-alpha-reductase inhibitors increase hair loss while using Nandrolone decanoate and other PED's. RU58841 is not as systemic as topical finasteride. Anyone have input on RU58841?

      It does work great but I use it in combination with finasteride. Finasteride deals with the DHT from testosterone and the RU helps with the other compounds

      No, it's much WORSE in terms of health. RU is an antiandrogen, meaining it will knock test and dht off the receptor.
      While finasteride side effect incidence after long term use has been shown to be the same as the plkacebo group

    I probably wouldn’t take finasteride whilst on arimidex. Until further study is done. One study not on humans showed that the prostrate increased in volume when taking both.

    I take finasteride for about 2 months every other year, that's what it works out to be. I notice my hair getting really fine and dry, which from my research say the follicles are suffering, and it brings it back to full and lush. I get no sides at all.

      Very interesting. I have never heard anyone using it like that.
      Please give us more info, like what dosage do you use, is this a common thing, is there a name for using it like this so i can find more info? How did you get the idea to try? And how did you know to stop after 2 months? Why every other year?
      Is that just when you feel it is needed or is there some known science why this is optimal?
      How long have you done this?
      And do you think using it topical 2 months every other year would work the same.
      How long is your hair? I guess how long you keep the hair should/could have something to with optimal length of dosing.

      @@cajampa Proscar is the brand, 5mg, 1 a day, 60 days. No science behind it, just taking advice from the old school guys on the forums of "little is more" only take enough to make it work. I tested it, it worked, I stopped and kept the results for a while. I take it whenever I see my hair change, it gets very fine and dry, nothing will help it, it seems like the follicles are dying, then I take fin and it fixes it for a while. This is my 4th run on it over about 7 years, it's working again and no sides, my hair is thick and lush again. I keep my hair short.
      Don't know anything about topical.

      @@JuggoJuggo Thanks, dude. OK but 5mg is not the common anti hair loss dose though. That is the shrink prostate dose. Most user are using 1mg. And already at 0.2mg you get 50% DHT reduction and at 5mg you only get up in the 68% range reduction of DHT.
      So 5mg sounds weird that is the opposite of less is more.
      More like as much as possible. Many are finding they are getting great results on only 0.25-0.5mg EOD.
      I think if it does work extra well for you because you keep your hair short.
      I like to keep my hair loooong, and if I would wait until I started to look thin it would take years to fill in.
      Fortunately for me oral minox and microneedling worked great.
      I think your strategy is probably optimal for one who keep the hair short, then you see the thinning easier and you get the full thickening back much faster also.
      But it is cool it can even work like this even though it sound very high to take 5mg every day. I mean the half life is 40 hours. That is why many are getting enough benefit at only 0.25-0.5mg EOD.

      @@threestans9096 Most hematocrit tests are done in the morning on an empty stomach, take that same person and fully hydrate them, or take the measurement in the evening if they're someone that drinks sufficient water, and poof, high hematocrit magically gone.

    Hi, im on 300mg testosterone a week, microdose twise a week (0.5mlx2) .. started taking finasteride, been on it 2/half weeks, im terrified of losing erections etc, now was taking 1mg daily, past few days ive noticed my downstairs is not as hard as usual and before taking finasteride id flick it or simply look at boobs and it would go up, now not so easy, would you recomend maybe every 2nd day trying that or ? Thanks

      Sounds like placebo. like this actually makes no sense and is your mind playing tricks on you.
      High T levels will prevent any type of side effect from finasteride

    I tried Finasteride I heard it doesn't do much for black people it ever says that on the label. I try DHT blockers results were inconclusive

    I don't have a hair lost at all I would say that my hair got thicker and I grew more on my chest and face. However I have quite bad ED and I'm only on 200mg a week and 75mg proviron over last week

      Estrogens to low because of proviron. With 250mg of test a week I don't need anything. Once I took anavar to gain a few pounds, it tanked my estrogens, I had less libido and ED issues. Went back to normal a few weeks after I stopped taking anavar. Proviron has a lowering effect on the estrogen as well.

    Fin should be Banned. I had horrible nightmares after just 3 doses. Causes brain changes and depression. Post Fin syndrome is real and ruins lives.

    If anyone is interested, there was a double blind, placebo controlled study on post finasteride syndrome. Of those who were given the placebo, a higher rate reported ongoing negative sexual side effects after stopping their medication (placebo) compared to the actual finasteride group. So, while it's true that around 1% of finasteride users do see adverse effects, there's not strong evidence for a type of post finasteride symptom that lasts for years after stopping the medication. The vast majority of people, like myself, have no noticeable adverse effects, and if you do have problems, stopping the drug will stop the associated negative effects.

      Thats quite interesting. Still DHT is a specific hormone, changing it's levels really should change how you feel. Not necessarily sexual dysfunction, but it should change you a bit
