I don't like how he is leaving work in his scrubs he has been wearing all day, potentially spreading germs everywhere. Surely against the policy of any hospital.
Because his character is not given a name. Maybe JD never learned his name. Maybe the janitor refuses to tell JD his name. The story is told from JD's perspective, so the janitor's "thoughts" are actually JD thinking what the janitor is thinking.
