It sounds like Tarrare may have had Prader-Willi syndrome. The disease is very rare and begins at about age 2 and they never feel full, they’re insatiable and get very upset when made to stop eating. It often overlaps with autism and possible chronic sensory under stimulation. The facial features often include thin lips too. I saw a documentary on it once and knew this sounded familiar. That must be awful for them and their caregivers. ♥️
Tarrare was not autistic, he was a genetic mistake, aside from his eating habits, his contemporaries saw no apparent signs of mental illness or unusual behaviour in him, other than an apparently apathetic temperament with "a complete lack of force and ideas". At the autopsy, Tarrare's gullet was found to be abnormally wide and when his jaws were opened, surgeons could see down a broad canal into the stomach. His body was found to be filled with pus, his liver and gallbladder were abnormally large, and his stomach was enormous, covered in ulcers and filling most of his abdominal cavity.

If I recall correctly, they did do an autopsy and found that his digestive system was abnormally large, as was his esophagus, but that also could be a chicken and the age situation (like was it always like that or did his insides stretch to accommodate everything?)
