Hey, man. You missed one fail phone call for Ray in TLAD. On Collector's Item, if you kill Issac, Johnny will call Ray and tell him that he will start looking for a new buyer.
Oh, and I forgot to mention for Out of Commission fail too in GTA IV. You can kill Jacob before or during the chase with Pegorino's Goons. (the fail cutscene from Niko, Jacob and Roman was duplicated) And maybe you should include fail cutscenes or phone calls variants as well, like Shadow, for example, where Niko has 2 variants when he kills the Dealer too early.

The best thing about failing a gta iv mission is seeing which car you get, it can either be a rare spawning car, common spawning car, or very rare- every time I get my one when failing, first thing I do is take it to the apartments and the garage
My theory is that at some point in development, Gracie was meant to die in the trunk of the Vincent while Niko was driving, and this would trigger the Ancelottis and Luis Lopez to go after him and Packie.
