Hmm. With the excellent touch-to-rack-focus function on all Sony cameras, what scenarios do you need manual focusing at all? Could you make a video on that topic? That would be awesome.

Totally fair, using a mini follow focus like this is a weird middle ground cause the most applicable use of a follow focus these days is to have an AC with a wireless system, and when a person is operating a small camera like this auto focus/using the touch screen is usually the way to go.
Personally I find myself using manual focus about 50% of the time for video with the a7siii. When I do use the follow focus it’s on something where I’m trying to be much more intentional, usually something like a video focussed on food or a still subject that I want to be very purposeful.
With that said it’s an excellent topic and I actually have a script for a larger video discussing the focusing modes for photos and I can make a follow up one on video!

Can you adjust the angle of the focus wheel up and down while connected to the gears, or is it fixed in place and depending on how it’s fixed to the rod?

you can control it independently however it can be rather finicky to do. The wheel also can be flipped to invert the angle it its at so you can have a better range of movement
