I know you said it would be letter d because of intramuscular but this is a little confusing because of the incorrect modifier since it was done on the left not right arm.

He actually missed it, it's not tumor yet, provider not confirmed that's why he said it's appeared to be benign, in last line it's sent to pathology for biopsy. So correct answer is C. Tell me if I'm missing anything i could be wrong as well.
