    Thank you Nacole for taking the time to do this and I plan I using your line tomorrow "Your plan of care for today is". You are right the patient would like to hear this because than they know they aren't forgotten from shift to shift.

    YOU ROCK! Even more beneficial and organized than many experienced RNs' reports I've heard. The fact that you stated you were a new grad just pump my confidence level! I'm a student nurse in my preceptorship and THANK YOU SO MUCH!

    IMO from this video I know you are without a doubt that you are a great nurse! I'm a new nurse myself working on a trauma floor and I thank you for this video! Keep up the wonderful work!

    This was good, imo. Good job keeping relief nurses abreast of 'to dos'. Way to be a team player. I'd rather have a long but thorough report vs gossip hour. I swear, I've never had to redirect a nurse so many times until last weekend. Like, 'Hey...FOCUS!' She was determined to turn every pt point into a discussion about some other coworker, her shoes or some other mundane topic. Drove me crazy.

    thank you nacole. you have been great and this video is by far the best video I have seen about off-shift report. I have been all over the place when giving report and I this video has shown me how to give a good detail report. continue what you're doing because new nurses like me really need a helping hand. thanks for all your help

    thank you so much. I am 44 and a student nurse, and when clinical instructor asks me to tell her about my patient...(give report), I freeze up. I'm trying to make a sheet to go off of, and your video made it a bit easier. Will be done in May, and I have to know how to do this correctly, and be able to communicate properly with the oncoming nurse.

    Great job Nacole. I'm a new nurse and end of shift reporting is a weakness for me. But I will get it. I love how you tie everything together.

    Thanks for the video!! I'm a new grad and just started my job last week. I'm trying to be more confident when it comes to giving report because right now it's terrifying :-P... Hoping it will get better once I get more comfortable at work.

    Nice end of shift report - however at no point do you mention involving the most important person in this report - The Patient - are you involving them asking their opinion? ensuring they know what is happening? In Australia we have National safety and quality standards - Standard 2 is Partnering with consumers recognising the importance of patients being actively involved in their care - Standard 6 Clinical Handover states we need to involve patients/ consumers in the shift to shift / transfer of care of patients in the process, This needs to be done with the patient and they need the opportunity to ask questions/ be actively involved in their care (unless unable to due to confusion etc) Involving the patient in your handover will significantly improve patient understanding and compliance

    Hi Nacole, I am a new nurse and would really like to be able to look in my patients charts and get more background info but there is hardly time for me to do all my charting along with patient care, how do you do it?

      +Jill Ellis I spend 15 minutes right after I do my first assessments looking at charts. They help me to get a big picture : )

      +Jill Ellis No, I do all my assessment first on all my patients... then look at the charts. I need to eyeball my patients are see where they are before I sit down and focus.

      Oh geeze, I goofeed, that makes sense to me now . I read that in a hurry and I was thinking you wrote first assessment (singular) and that did not make sense to me. Sorry! Ugh, I need to read slower and wear my glasses. Makes much more sense now! Thanks again Nacole!
