I'd add that the Far Realm is a place where the PCs should not ever return from unscathed, if they return at all. So if that is the sort of thing a DM wants to avoid, they probably want to stick to having the adventure take place in a region affected by the Far Realm through rifts and such, rather than going into the plane itself.
